侧脑室微创穿刺引流联合亚低温治疗脑室出血的临床疗效

摘　　要：目的观察侧脑室微创穿刺引流联合亚低温治疗脑室出血的临床疗效。方法将2009年6月-2012年5月入住合肥市第二人民医院治疗的脑室出血患者根据匹配条件分为两组:对照组行内科保守治疗;治疗组在内科保守治疗的同时行侧脑室微创穿刺引流联合亚低温治疗。比较两组治疗前后21 d神经功能缺损评分(CSS)和随访3个月时的日常生活能力(Barthel指数)。结果治疗组有效率高于对照组(72.3%VS 32.6%,P<0.05);两组治疗前后神经功能缺损评分差异有统计学意义(P<0.05);治疗3个月后Barthel指数治疗组为(81.6±7.2)分,大于内科对照组的(55.2±6.4)分,差异有统计学意义(P<0.05)。结论侧脑室微创穿刺引流联合亚低温治疗脑室出血较传统内科治疗有效,且有利于神经功能恢复。Objective To observe the clinical effects of minimally invasive surgery for hematoma evacuation via the lateral ventricle combined with local mild hypothermia in treatment of intraventricular hemorrhage.Methods Total 62 patients with ventricular hemorrhage in our hospital from Jun.2009 to May 2012 were divided into observation group（30 cases） and control group（32 cases）.The conservative internal medicines were performed in both groups.While the observation group received local mild hypothermia treatment after minimally invasive surgery for hematoma evacuation via the lateral ventricle.The therapeutic effect was compared between the two groups.Results Effective rate,mortality rate and rehemorrhage rate of the observation group were 73.3%,10.0% and 10.0%,respectively,while of the control group were 43.8%,37.5% and 12.5%,respectively.The difference in the effective rate and mortality rate were significant between the two groups（P〈0.05）,but the rehemorrhage rate not（P〈0.05）.Conclusion The effect of minimally invasive surgery for hematoma evacuation via the lateral ventricle combined with local mild hypothermia for intraventricular hemorrhage is more obvious as compared to the routine medical treatment.The recovery of neural function is certain.
