急性脑梗死的TCD与CT诊断对比分析

摘　　要：目的分析急性脑梗死的经颅多普勒超声(TCD)与CT诊断结果,比较两者之间的差异。方法选择荆州市第一人民医院2008年8月-2012年7月收治的急性脑梗死患者160例,均经颅脑CT检查确诊,按照梗死面积分为三组。采用经颅多普勒超声诊断仪进行TCD检查,观察各组患者TCD检查结果的异常率。结果急性脑梗死患者TCD异常率为71.88%(115/160)。其中大面积梗死患者TCD异常率为91.38%(53/58);小面积脑梗死患者TCD异常率为61.82%(34/55);腔隙性脑梗死患者TCD检查率为59.57%(28/47),其中大面积梗死患者TCD异常率明显较高,差异有统计学意义(P<0.05)。结论 TCD检查可直接获得患者脑动脉血流动力参数,对急性脑梗死尤其是大面积梗死不失为是一种重要的辅助诊断方法。Objective To compare the results of transcranial Doppler（TCD） and computed tomography（CT） in the diagnosis of acute cerebral infarction（ACI）.Methods Total 160 patients with ACI confirmed by brain CT from August 2008 to July 2012 in our hospital were divided into three groups： large infarction,small infarction and lacunar infarction group.TCD was performed in all patients to observe the rate of abnormality.Results The total abnormal rate of TCD examination in ACI patients was 71.88%,with a rate of 91.38% in the large infarction group,61.82% in the small infarction group and 59.57% in the lacunar infarction group.The abnormal rate of TCD in the large infarction group was more obviously.The difference among the three groups was statistical significant（P〈0.05）.Conclusion The cerebral artery hemodynamic parameters can be directly got by TCD examination which will be an important auxiliary diagnostic method for acute cerebral infarction especially the large area cerebral infarction.
