血清胰蛋白酶原-2与急性胰腺炎严重程度分析

摘　　要：目的研究血清胰蛋白酶原-2与急性胰腺炎严重程度的相关性。方法收集中国人民解放军北京军区总医院2009年12月~2011年6月及北京市石景山医院2011年6月~2012年2月内、外科发病〈12h、未经生长抑素治疗的急性胰腺炎患者血清74例,正常健康人血清85例,采用ELISA法半定量检测血清中胰蛋白酶原-2的含量。结果健康人血清胰蛋白酶原-2含量中位数为：0.9ng/ml,急性胰腺炎组血清胰蛋白酶原-2中位数为：83.70ng/ml。急性胰腺炎组、正常对照组比较差异有统计学意义（P〈0.01）,散点图提示血清胰蛋白酶原-2值与Balthazar CT严重指数之间存在正相关,采用Spearman秩相关提示二者存在正相关（P〈0.01）,相关系数为0.485。结论血清胰蛋白酶原-2数值与急性胰腺炎严重程度呈正相关,ELISA法简便、无创,可用于早期判断急性胰腺炎严重程度。Objective To evaluate the correlation between the content of serum trypsinogen-2 and the severity of acute pancreatitis.Methods Fresh serum samples were collected from 74 patients with acute pancreatitis without prior treatment with somatostatin,and 85 normal controls.Serum levels of trypsinogen-2 were measured by ELISA.Resutls The median level of serum trypsinogen-2 was significantly higher in patients with acute pancreatitis than that of normal controls（83.7μg/L,vs 0.9μg/L,P 0.01）showing a positive correlation between the content of serum trypsinogen-2 and CT severity index in acute pancreatitis patients from scatterplot,and that was further confirmed by Spearman rank correlation（r=0.485,P0.01）.Conclusion There was positive correlation between the content of serum trypsinogen-2 and the severity of acute pancreatitis.It could be used for judgement of the severity of acute pancreatitis.
