胃癌患者循环单核样髓源抑制性细胞的表达及其临床意义

摘　　要：目的探讨胃癌患者外周血髓源抑制性细胞(MDSCs)的表达及临床意义。方法采用流式细胞术检测77例胃癌(A组)、21例胃上皮内瘤变(B组)、31例萎缩性胃炎(C组)、45例胃溃疡(D组)和20例正常对照(E组)外周血MDSCs占单核样细胞百分比,并进行分层分析。结果 A、B、C、D和E组外周血MDSCs占单核样细胞百分比分别为(21.72±10.12)%、(13.16±3.79)%、(7.74±1.14)%、(4.79±1.07)%和(2.90±1.80)%,组间差异均有统计学意义(P<0.01)。进展期胃癌MDSCs(23.79±9.48)%,较早期胃癌MDSCs(11.74±4.01)%水平明显升高(P<0.05);未观察到MDSCs比例高低与患者肿瘤浸润深度、淋巴结转移有关。结论胃癌患者外周血MDSCs水平的升高与胃癌免疫功能低下及肿瘤的发展密切相关。Objective To explore the expression and clinical significance of peripheral monocytic myeloid derived suppressor celts（MDSCs） in patients with gastric cancer. Methods The percentages of MDSCs in the monocytes in peripheral blood were calculated in 77 patients with gastric cancer（group A）, 21 cases with gastric intraepithelial neoplasia （group B）, 31 cases with atrophic gastritis（group C）, 45 cases with gastric ulcer（group D） and 20 healthy controls（group E）. The stratified analysis was then performed. Results The percentages of MDSCs in the monoeytes for groups of A,B,C,D and E were （21.72±10. 12）%, （13. 164±3.79）%, （7.74±1.14）%, （4.79± 1.07）~ and （2.90 ± 1.80）%, respectively. The differences among 5 groups were significant （P〈0. 01）. The level of MDSCs was higher in the cases with advanced gastric cancer than that in those with early gastric cancer （P〈0. 05）. No significant correlation was seen between MDSCs expression and tumor infiltration and lymph node metastasis. Conclusion An increase of MDSCs expression in the peripheral blood of the patients with gastric cancer is closely related to the suppression of the immune function and progress of gastric cancer.
