摘  要:目的评估64层螺旋CT双动脉期扫描对肝局灶性结节增生(FNH)的诊断价值。方法收集12例(14个病灶)FNH患者的CT资料。肝脏64层螺旋CT扫描采用双动脉期、门静脉期三期增强扫描。采用对比剂追踪触发技术启动动脉早期扫描,触发监测点设为腹腔干层面的降主动脉,触发阈值140HU,触发后延迟5S开始动脉早期扫描,延迟20S开始动脉晚期扫描,延迟50S开始门静脉期扫描;薄层多平面重建图像。分析CT双动脉期病灶的强化特点和动脉早期、动脉晚期显示病灶供血动脉的能力。结果9个病灶动脉早期强化程度高于动脉晚期.5个病灶动脉早期强化程度低于动脉晚期;有10个病灶可见供血动脉,其中7个病灶仅在动脉早期显示,3个病灶在动脉早期和动脉晚期均能显示。结论多层螺旋CT双动脉期扫描提高对FNH的供血动脉的显示,对FNH的诊断具有特征性。Objective To investigate the value of 64-slice CT contrast enhanced dual arterial phase in the diagnosis of focal nodu- lar hyperplasia (FNH). Methods 12 patients(14 lesions) with FNH underwent contrast enhanced 64-slice helical CT examinations, including dual arterial phase and portal phase. The early arteries phase was initiated by automatic bolus tracking 5 s delay after trig- gering at a threshold of 140 HU in the aorta at the origin of the celiac arteries, the late arteries phase 20 s delay after triggering,and the portal phase 50 s delay after triggering. Then multi-direction MPR was performed to display the supply arteries of lesion and the imaging features of dual arterial phase were analyzed comparatively. Results In the early arterial phases, the lesion showed more en- hancement than that in the late arteries phase in 9 cases and showed less enhancement in 5 cases. The supply arteries were seen in 10 cases,in which 7 cases appeared only in the early arteries phase and 3 cases appeared in the early arterial phases and the late arterial phases. Conclusion Muhi-slice CT contrast-enhanced dual arterial phase can improve the detecting rate of the supply arteries of FNH,and is specific and accurate in diagnosing FNH.
