成人型肺母细胞瘤的CT及PET/CT表现

摘　　要：目的探讨成人型肺母细胞瘤(ATPB)的CT及PET/CT表现,提高对其诊断的准确率。方法对9例经手术或穿刺病理证实的ATPB的影像学表现进行回顾性分析。MSCT多期增强扫描检查8例,PET/CT检查4例,其中3例同时做CT及PET/CT检查。结果 9例ATPB均为周围型,均为单发,其中1例合并肺内多发转移。CT表现:8例呈边缘光滑锐利或仅有浅分叶的圆形或类圆形结节或肿块,仅1例可见典型浅分叶、毛刺及胸膜牵拉征象。5例病灶直径≥5 cm且密度不均,7例有坏死液化,增强扫描强化明显。18F-FDG PET/CT显像4例病灶呈均匀或不均匀放射性浓聚,标准摄取值最大值约3.2～4.7。结论 ATPB影像学上缺乏特征性表现,综合CT和PET/CT影像学检查能提高诊断符合率,对部分病例可做出提示性诊断。Objective To improve diagnostic accuracy of adult type pulmonary blastoma（ATPB） by evaluating the ap- pearances of CT and PET/CT. Methods The image data of 9 patients with ATPB confirmed by surgical and pathological results was retrospectively analyzed, there was 8 cases with dynamic enhanced MSCT and 4 cases with PET/CT. Results All 9 cases exhibited a single lesion and peripheral type. CT findings included： 8 cases had solitary oval shaped nodules or masses with distinct boundaries or mild lobulated lesions, 1 had typical lobulated lesions, burr sign and hollowed pleura. There were 5 cases with more than 5cm in diameters. Necrosis of tumors was seen in 7 cases. The lesions had obvious en- hancement; PET/CT findings included： 4 cases showed homogeneously or inhomogeneously increased radioactivity uptake （ SUVmax 3.2 - 4.7）. Conclusion ATPB is lack of characteristic imaging features, and suggestive diagnosis can be made in some patients combined with CT and PET/CT.
