乙酰肝素酶、CD222在膀胱癌中的表达及其临床意义

摘　　要：目的探讨膀胱癌中乙酰肝素酶和CD222的表达水平及其与膀胱癌发生、发展、转移及预后的关系。方法应用免疫组化SP法研究95例膀胱癌标本中乙酰肝素酶和CD222的表达水平。采用χ2检验、Kaplan-Meier法、Logrank检验等统计学方法分析乙酰肝素酶、CD222的表达水平与膀胱癌临床病理特征及5年生存率的关系。结果乙酰肝素酶在膀胱癌中阳性表达65例(68.42%),其表达率与膀胱癌患者的肿瘤直径、病理分级、病理学分期、淋巴结转移等情况明显相关(P<0.05)。CD222在膀胱癌中阳性表达58例(61.05%),其表达率与膀胱癌患者的肿瘤病理分级、病理学分期、淋巴结转移等情况明显相关(P<0.05)。乙酰肝素酶和CD222的表达呈正相关(P<0.05)。生存曲线分析显示乙酰肝素酶阳性表达组患者的5年生存率明显低于阴性表达组。CD222阳性表达组患者与其阴性组相比5年生存率无明显差异(P>0.05)。乙酰肝素酶、CD222共表达阳性组与其阴性组比较,5年生存率有统计学意义(P<0.05)。结论乙酰肝素酶及CD222在膀胱尿路上皮癌中的表达与肿瘤的发生、发展、转移和预后相关。其中乙酰肝素酶及CD222的共表达对膀胱癌生存预后的预测更具参考价值。Objective To study the expressions of heparanase and CD222 in bladder cancer and tiaelr relatlonsmp With me tumor development, progression, metastasis and prognosis. Methods Immunohistochemical technique was used to detect the expression of heparanase and CD222 in 95 cases of bladder cancer. Their relation to the clinicopathological features and five- year survival rate of bladder cancer was analyzed by Chi-squafe test, Kaplan-Meier method and logrank test. Results The positive protein expression of heparanase was 68.42% （65/95）. The heparanse protein expression was correlated with the turnout diameter, pathological grade, pathological stage and lymph node metastasis （P〈0.05）. The positive protein expression of CD222 was 61.05% （58/95）. The CD222 protein expression was correlated with the tumor pathological grade, pathological stage and lymph node metastasis （P〈0. 05）. A positive correlation was observed between the expression of heparanase and that of CD222 （P〈0.05）. The survival curve analysis demonstrated that 5-year survival rate was significantly lower in heparanase positive group than negative group （P〈0.05）. And the positive protein co-expression of heparanase and CD222 group had low- er 5-year survival rate than the negative group （P〈0.05）. GOnCluSiOnS The expressions of heparanase and CD222 in blad- der cancer were correlated with the tumor development, progression, metastasis and prognosis. And the co-expression of heparanase and CD222 may be a better predicator for the prognosis of bladder cancer.
