摘 要:目的肿瘤的转移涉及多种基因及其产物的参与,其中转移相关基因1(metastasis-associated 1,MTA1)基因是最近被发现的一个肿瘤转移相关基因。文中拟探讨MTA1蛋白在非小细胞肺癌(non-small cell lung cancer,NSCLC)组织中表达及与患者临床病理特征之间的关系,分析其对判断NSCLC患者转移潜能和预后的意义。方法采用免疫组化染色法检测96例具有完整随访资料的NSCLC组织标本中MTA1蛋白的表达,同时结合临床病理特征进行分析,明确其与转移及预后的相关性。结果在NSCLC组织中,MTA1蛋白表达率在不同年龄、性别、病理类型间差异无统计学意义(P>0.05),而在不同肿瘤直径(d)(d≤3 cm 54.5%、3 cm<d≤5 cm 93.3%与d>5 cm 81.8%,χ2=13.898,P=0.001)、TNM分期(Ⅰ期53.8%、Ⅱ期46.7%、Ⅲ期89.1%与Ⅳ期81.8%,χ2=15.348 P=0.002)、分化程度(高中分化61.5%,低未分化81.4%,χ2=4.117 P=0.042)、淋巴结转移与否(59.1%与81.1%,χ2=4.501 P=0.034)间有明显差异。生存曲线分析表明MTA1高表达的NSCLC患者的2年的无疾病进展(progress free survival,PFS)发生率为8.2%,低表达者2年PFS发生率为30.4%(P=0.007)。Cox多因素分析显示,MTA1蛋白的高表达、肿瘤TNM分期、淋巴结转移是影响患者预后的独立风险因素。结论MTA1蛋白在NSCLC组织中呈高表达,且与肺癌转移及预后密切相关。Objective Cancer metastasis involves multiple genes and their products. Metastasis-associated-gene 1 ( MTA1 ) is a newly discovered cancer metastasis-related gene. This study was to explore the relationship of MTA1 expression in non-small cell lung cancer (NSCLC) with the clinicopathological profiles of the patient and determine its value in predicting the metastasis potential and clin- ical prognosis of NSCLC. Methods Using immunohistochemical staining, we detected the expression of MTA1 in the NSCLC tissue samples of 96 patients with complete follow-up data. Meanwhile, we analyzed the clinicopathological profiles and evaluated their correla- tion with metastasis potential and prognosis of NSCLC. Results The expression of MTA1 exhibited significant differences in NSCLC a- mong tumors of different diameters (d) ( d ≤ 3 cm : 54.5 %, d 〈_5 cm : 93.3%, d 〉 5 cm : 81.8%, X2 = 13. 898, P = 0.001 ), TNM sta- ges ( stage I: 53.8%, stage II: 46.7%, stage m: 89.1%, stage IV: 81.8%, X2 = 15. 348, P = O. 002), differentiation degrees ( high and moderate: 61.5% , low and anaplastie: 81.4%, X2 =4.117, P = 0.042) , and between lymph node metastasis and non-lymph node metastasis (59.1% vs 81.1%, X2 =4. 501, P =0.034), but not among different age and gender groups and pathological types (P 〉 0.05 ). The Kaplan-Meiert survival curve showed the 2-year the progress-free survival (PFS) rates to be 8.2% and 30.4% in the NSCLC patients with highly and lowly expressed MTA1, respectively (P =0. 007). COX multivariate analysis indicated that high MTA1 expres- sion, TNM staging and lymph node metastasis were independent risk factors for the prognosis of NSCLC. Conclusion MTA1 is highly expressed in NSCLC, and is closely associated with the metastasis and prognosis of lung cancer.
