摘 要:目的研究碘海醇(欧乃派克)复合染料定位兔舌癌前哨淋巴结的可行性。方法健康成年新西兰白兔14只,分为正常对照组(A组)3只、兔颈淋巴结炎性肿大模型组(B组)8只和兔VX2舌癌颈淋巴结转移模型组(C组)3只。B组6只和C组3只采用碘海醇和亚甲蓝(美蓝)混合液行间接淋巴造影,B组另外2只采用碘海醇和纳米炭混合液行间接淋巴造影。根据CT扫描和染色定位前哨淋巴结。结果兔一侧舌腹外侧注射碘海醇后,只有同侧淋巴管和颈淋巴结增强显影。注射造影剂后双侧淋巴管及颈部淋巴结明显增强显影。在所有实验兔中,每侧颈部仅有一枚淋巴结增强显影,增强显影的淋巴结位于喉气管旁。在CT扫描定位指引下,采用碘海醇和亚甲蓝混合液造影的9只兔均在喉气管旁找到一枚蓝染淋巴结,另外2只采用碘海醇和纳米炭混合液造影的兔也均在喉气管旁找到一枚黑染淋巴结。该蓝染或黑染淋巴结实际位置与间接淋巴造影CT位置一致。结论碘海醇复合染料定位可准确定位兔舌前哨淋巴结。Objective To investigate the feasibility of localizing sentinel lymph node of rabbit tongue cancer by using iohexol(omnipaque) composite staining. Methods In this study, 14 adult New Zealand rabbits in good health were pre-treated and divided into three groups, including the control group (group A, containing 3 rabbits), cervical lymph node inflammatory swelling model group(group B, containing 8 rabbits), and rabbit VX2 tongue cancer with cervical lymph node metastasis model group (group C, containing 3 rabbits). According to the experimental design, rabbits in group B were divided into subgroup I and subgroup II, which contained 6 rabbits and 2 rabbits respectively. For subgroup I of group B and group C, the mixture of iohexol and methylene blue was used as contrast agent of indirect lymphography. For subgroup II of group B,the mixture of iohexol and nano-carbon was used as contrast agent of indirect lymphography. Subsequently, the computed tomography lymphography and dyeing was combined for mapping sentinel lymph node of rabbit tongue cancer. Results At one side of ventral tongue where iohexol was injected, the enhanced developing only appeared in the lymphatic and cervical lymph nodes at the same side. However, remarkably enhanced developings were observed in both bilateral lymphatic and cervical lymph nodes after injecting the contrast agents. There was only one enhance developing area of lymph node at one side of the neck in each rabbit. Using CT scan localization, a bluestained lymph node at larynx trachea of each rabbit injected with the mixture of iohexol and methylene blue was found. A blackstained lymph node at larynx trachea of each rabbit injected with the mixture of iohexol and nanocarbon was also found. The location of eithers bluestained or blackstained lymph nodes was well matched with that detected with CT scan localization. Conclusions The approach of iohexol composite staining was practicably accurate for localizing sentinel lymph node of rabbit tongue.
