即刻种植和延期种植对种植体周围软组织影响的对比研究

摘　　要：目的:对比研究前牙区即刻种植和延期种植技术对前牙区种植体周围软组织的影响。方法:48例前牙缺失病例,A组23例:行不翻瓣拔牙即刻植入30颗ITI种植体。B组25例:行延期种植植入ITI种植体30颗。A、B两组在种植体愈合3～6个月后均进行氧化锆全瓷冠永久修复。基台连接后的1、3、6、9、12个月时检测种植体周改良龈沟出血指数(mBI)、探诊深度(PD),并测定红色美学评分(pink esthetic score,PES)。采用SPSS 17.0进行实验数据统计学分析。结果:基台连接后1个月时,即刻种植组mBI高于延期种植组(P<0.05)。PD值在基台连接1、3、6、9、12个月时2组均无统计学差异(P>0.05),PES值在永久修复3个月时有统计学差异(P<0.05)。结论:延期种植、即刻种植对种植体周围软组织无明显有害影响,即刻种植在永久修复3个月时的红色美学效果要优于延期种植。Objective：To study the influences of immediate implant or delayed implant on peri-implant soft tissues in anterior teeth area.Methods： 48 patients with anterior teeth missing were included.23 cases in group A received flapless 30 immediate ITI implants altogether.25 cases in group B received 30 delayed implants.Zirconia crowns were applied on the implants 3-6 months after implantation.1,3,6,9 and 12 months after the abutment connection the peri-implant soft tissues were examined and evaluated by modification sulcus bleeding index（mBI）,probing depth（PD） and pink esthetic score（PES） of the soft tissue.Analysis of the data was performed by software SPSS 17.0.Results： The mBI in the patients with immediate implant was higher than that in those with delayed implant in the first month after abutment connection（P〈0.05）.The PD of the 2 groups showed no significant difference at every test time after abutment connection（P〈0.05）.PES in the patients with immediate implant was higher than in those with delayed at the time of three months after permanent restoration（P〈0.05）.Conclusion： Immediate implant and delayed implant have similar harmless effect,however immediate implant has better pink esthetic effects at the third month after restoration.
