摘  要:目的:检测正畸治疗患者口腔中的具核梭杆菌(Fn)及毒力因子FadA粘附素。方法 :随机收集120例患者的临床标本分别组成正畸治疗组(A组55例)、正常对照组(B组30例)、牙周炎组(C组35例),记录其各自GI,进行细菌DNA提取及16SrDNA PCR反应,并与牙龈指数GI进行相关性分析。结果:120例患者的临床标本中扩增出Fn 81株,检出率67.50%,A、B、C组检出率分别为69.09%、46.67%和82.86%(χ2=9.756,P<0.01);以FadA引物直接扩增其FadA基因片段,扩增出67例,阳性率55.83%,A、B、C组检出率分别为58.18%、33.33%和71.43%(χ2=63.53,P<0.005);经Spearman等级相关分析得出:正畸治疗组中Fn及FadA的检出率与GI之间存在明显的正相关关系(P<0.05)。结论:Fn与正畸治疗中牙龈炎性反应的发生发展密切相关;毒力因子FadA粘附素的检出率较高,在Fn致病机制中可能起重要作用。Objective:To detect Fusobacterium nucleatum(Fn)and its virulence factor FadA adhesin in the patients with orthodontic treatment.Methods: Samples were collected from 120 patients and divided into 3 groups: orthodontic treatment group(A,n=55),control group(B,n=30) and periodontitis group(C,n=35).The GI of each patient was recorded.Bacteria DNA was extracted and the gene expression was examined by 16Sr DNA PCR technique.The correlation between the Fn/or FadA and gingivitis index(GI) was studied by spearman rank correlation analysis.Results: Fn positive ratio of all samples was 67.50%(81/120).In group A,B and C was 69.09%,46.67% and 82.86% respectively(χ^2=9.756,P〈0.01).FadA positive ratio of all samples was 55.83%(67/120),in group A,B and C was 58.18%,33.33% and 71.43% respectively(χ^2=63.53,P〈0.005).Positive correlation(P〈0.05)was observed between GI and the prevalence of Fn and/or FadA adhesin.Conclusion: Fn is closely related the to occurrence and development of gingivitis reaction during orthodontic treatment.FadA adhesin may play an important role in the pathogenic mechanism of Fn.
