声辐射力脉冲成像评价肝弹性在布加综合征治疗中的意义

摘　　要：目的探讨声辐射力脉冲成像评价肝弹性在布加综合征治疗中的应用价值。方法18例行球囊扩张术的布加氏综合征患者，于术前2d、术后2d、术后3个月及术后6个月利用声辐射力脉冲成像技术进行肝弹性测定，分析其变化情况。结果16例患者成功进行球囊扩张术，患者治疗前后不同时间点肝弹性分别为（3．06±0．53）m／s、（2．13±0．50）m／s、（1．89±0．29）m／s及（1．96±0．46）m／s，治疗后2d及3个月时其弹性指数与前次测定相比差异具有统计学意义（P〈0．01）。结论声辐射力脉冲成像测定的肝弹性值随静脉再通而明显降低，在监测布加氏综合征的治疗效果、术后复发等方面具有一定I临床意义。Objective To discuss the value of liver stiffness measured using acoustic radiation force impulse imaging in the treatment of patients with Budd-Chiari syndrome. Methods Eighteen patients were enrolled in this study. All the patients were performed percutaneous luminal angioplasty. Liver stiffness was measured 2 days before,2 days after,3 months after and 6 months after the treatment,and the difference of liver stiffness was analyzed. Results Sixteen patients achieved successful recanalization and underwent significant decrease of liver stiffness after angioplasty 2 days and 3 months after the treatment [ P d0.01, with the liver stiffness of （3.06±0. 53）m/s,（2.13±0.50）m/s, （1.89± 0.29）m/s and （1.96±0.46）m/s respectively in the four time points]. Percutaneous luminal angioplasty failed in two patients and there was no significant decrease of liver stiffness in those patients. Recurrence of obstruction after successful recanalization occurred in one patient, and the liver stiffness significantly rose （ P 〈0.01）. Conclusions The value of liver stiffness measured using acoustic radiation force impulse imaging significantly decreased after angioplasty and is considered to be valuable in reflecting the recanalization of hepatic vein and recurrence of obstruction.
