同步放化疗联合内生场热疗治疗局部晚期非小细胞肺癌

摘　　要：目的观察同步放化疗联合内生场热疗治疗局部晚期非小细胞肺癌(NSCLC)的临床疗效和耐受性。方法 74例不能手术的局部晚期NSCLC患者随机分为同步放化疗联合热疗组(A组)和同步放化疗组(B组)。A组于放化疗的同时再给予内生场热疗,每周2次,直至全部放化疗结束。比较两组疗效及不良反应。结果两组近期疗效分别为88.6%和79.4%;1年生存率为77.1%和67.6%,中位生存期为(21±1.214)个月和(17±2.309)个月,P>0.05。不良反应中Ⅲ～Ⅳ级骨髓抑制、放射性食管炎、放射性肺炎在A、B组分别为54.3%和79.4%;28.6%和52.9%;5.7%和23.5%,P<0.05。结论以3DCRT为基础的同步放化疗联合内生场热疗治疗局部晚期NSCLC疗效较好,患者可耐受,而且热疗能明显减轻同步放化疗引起的毒副反应。Objective To observe the clinical effect and tolerance of the concurrent chemoradiotherapy combined with endogenetic field tumor hyperthermia （EFTH） in the treatment of locally advanced non-small cell lung cancer （NSCLC）. Methods 74 patients with inoperable locally advanced NSCLC were randomly divided into the concurrent chemoradiotherapy combined with hyperthermia group （group A） and the concurrent chemoradiotherapy group （group B）. Group A was given endogenetic field thermotherapy 2 times per week at the same time of chemoradiotherapy until the end of therapy. Results The efficacy rate in group A and group B was 88.6% and 79.4% respectively. The 1 year survival rate and the median survival time of the patients in group A and group B were 77.1% vs. 67.6% and （21 ± 1.214） months vs. 07 ± 2.309） months, respectively, P〉0.05. III - IV grade myelosuppression, radiation esophagitis, radiation pneumonitis in group A, group B were 54.3% vs. 79.4%, 28.6% vs. 52.9% and 5.7% vs. 23.5%, respectively （P〈0.05）. Conclusions Preliminary results show that the 3DCRT based concurrent chemoradiotherapy plus endogenetic field tumor hyperthermia for locally advanced non-small cell lung cancer is effective and well tolerated. Hyperthermia significantly lessens the toxicities of concurrent chemoradiotherapy.
