急性脑梗死患者血清3项指标水平分析

摘　　要：目的检测分析急性脑梗死患者血清脂蛋白相关磷脂酶A2(Lp-PLA2)、超敏C反应蛋白(hs-CRP)及脂蛋白(a)[LP(a)]水平变化及临床意义。方法急性脑梗死患者100例(急性脑梗死组)与同期体检健康者40例(健康对照组),平行检测两组血清Lp-PLA2、hs-CRP及LP(a)水平。急性梗死组按美国国立卫生研究院卒中量表(NIHSS)评分,比较不同NIHSS评分患者的血清Lp-PLA2、hs-CRP及LP(a)水平。结果急性脑梗死组血清Lp-PLA2、hs-CRP及LP(a)水平明显高于健康对照组(P<0.01),且各指标水平变化呈显著正相关(r分别为0.339,0.545,0.297,P<0.01)。急性脑梗死组NIHSS评分小于5分、5～15分和大于15分3个亚组之间血清Lp-PLA2、hs-CRP及LP(a)水平差异有统计学意义(P<0.05)。NIHSS评分越高的亚组,其血清Lp-PLA2、hs-CRP及LP(a)水平越高。血清Lp-PLA2、hs-CRP及LP(a)水平与急性脑梗死患者病情严重程度均呈高度正相关(r分别为0.745、0.636、0.701,P<0.01)。结论检测急性脑梗死患者Lp-PLA2、hs-CRP及LP(a)血清水平,对临床诊断有一定参考价值。Objective To detect and analyze the levels and clinical significance of serum lipoprotein-associated phospholipase A2（Lp-LA2）,high sensitivity C-reactive protein（hs-CRP） and lipoprotein（a） in aacute cerebral infarction patients.Methods 100 patients with acute cerebral infarction（acute cerebral infarction group） and 40 cases of physical healthy population over the same period（control group） were selected.The levels of serum Lp-PLA2,hs-CRP and Lp（a） were detected of two groups.Acute cerebral infarction group were assessed by national institutes of health stroke scale（NIHSS）.The levels of serum Lp-PLA2,hs-CRP and Lp（a） were compared among the patients with different NIHSS scores.Results The levels of serum Lp-PLA2,hs-CRP and Lp（a） in acute cerebral infarction group were significantly higher than the control group（P0.01）,and each index levels change was positively correlated（r=0.339,0.545,0.297,P0.01）.The differences of serum Lp-PLA2,hs-CRP and Lp（a） levels were statistical significance among NIHSS 5,5~15 and 15 score sub-groups（P0.05）.The higher scores of NIHSS,the higher of the serum Lp-PLA2,hs-CRP and Lp（a） levels.The levels of Lp-PLA2,hs-CRP and Lp（a） were positively correlated with acute cerebral infarction degree（r=0.745,0.636,0.701,P0.01）.Conclusion There might be some reference values for clinical diagnosis by detecting the levels of serum Lp-PLA2,hs-CRP and Lp（a） in acute cerebral infarction.
