舍曲林合并奥氮平治疗强迫性障碍临床对照研究

摘　　要：目的探讨舍曲林合并奥氮平治疗强迫性障碍的疗效及安全性。方法将74名住院强迫性障碍患者随机分为研究组和对照组各37例，分别使用舍曲林＋奥氮平和单用舍曲林治疗，进行为期8周开放式研究。舍曲林起始剂量50mg·d^-1，剂量范围100～200mg·d^-1；奥氮平起始剂量2．5mg·d^-1，剂量范围5～10mg·d^-1。在治疗第2、4、6、8周末使用耶鲁-布朗强迫量表（Y．BOCS）和汉密尔顿焦虑量表（HAMA）评定疗效，用副反应量表（TESS）评定不良反应。结果治疗8周后，两组Y-BOCS、HAMA评分均比治疗前显著降低（均P〈0．05）。且组间比较有显著差异（P〈0．05）。研究组有效率显著高于对照组（68％VS．40％，P〈0．05）。研究组体重增加发生率高于对照组（22％VS．5％，P〈0．05），其他各项不良反应发生率无显著差异（P〉0．05）；TESS量表总评分组间无显著差异（P〉0．05）。结论奥氮平联用舍曲林治疗强迫性障碍疗效显著，安全性佳。AIM To investigate the efficacy and safety of sertraline combined olanzapine in the treatment of obsessivecompulsive disorder （OCD） patients. METHODS Seventy-four inpatients diagnosed with OCD were randomly divided into two groups （n = 37 for each group） ： study group received sertraline combined olanzapine therapy and control group received single sertraline therapy in the 8-week treatment. Starting dose of sertraline was 50 mg.d-1 and the dose range was 100 - 200 mg.d-1. Starting dose of olanzapine was 2.5 mg＇d-1 and the dose range was 5 - 10 mg.d-1. At the end of week 2, 4, 6, 8, the efficacy was assessed with the Yale- Brown Obsessive Compulsive Scale （Y- BOCS） and Hamilton Anxiety Rating Scale （ HAMA）, and the adverse reactions were assessed by the Treatment Emergent Symptom Scale （TESS）. RESULTS After the 8- week treatment, Y- BOCS and HAMA scores were significantly decreased in both two groups （P 〈 0.05） compared with those before the treatment, with significant differences between the two groups （P 〈 0.05）. Effective rate of the study group was higher than that of the control group （68% vs. 40%, P 〈 0.05）. The rate of weight gain of study group was higher than that of the control group （22% vs. 5%, P 〈 0.05）, and other adverse reactions had no statistical differences （P 〉 0.05）. TESS scores didn＇t show statistical difference between the two groups （P 〉 0.05）. CONCLUSION Sertraline combined olanzapine is effective and safe in the treatment of OCD patients.
