阿维A联合CO_2激光治疗复发性尖锐湿疣30例临床观察

摘　　要：目的观察阿维A联合CO2激光治疗复发性尖锐湿疣(CA)的临床疗效及其对患者外周血T淋巴细胞亚群的影响。方法将入选的61例复发性CA患者分为治疗组30例,对照组31例,全部患者予CO2激光清除皮损,另外治疗组还予口服阿维A胶囊10mg,2次/d,连用1个月。随访3个月,观察治疗前、后患者外周血T细胞亚群变化情况。结果治疗组治疗后患者的CD3+,CD4+T细胞、CD4+/CD8+比值均高于治疗前,但是患者的CD8+T细胞值低于治疗后,以上差异均有统计学意义(P均<0.05)。还显示对照组T淋巴细胞各值治疗前、后的差异均无统计学意义(P均>0.05)。治疗结束后,治疗组复发率(20.00%,6/30)明显低于对照组(54.84%,17/31);而且治疗组CD3+,CD4+T细胞升高、CD8+T细胞下降情况均优于对照组,差异也均有统计学意义(P均<0.05)。结论阿维A联合CO2激光治疗CA能有效降低复发率。其降低复发率可能与阿维A调节患者外周血T淋巴细胞亚群功能相关。Objective To explore the therapy effect and it＇s influcence on T-lymphocyte subpopulation of acitretin com- bined with carbon dioxide laser on relapsed patients with eondyloma aeuminatum（CA）. Methods A total of 61 relapsed patients with CA were randomly divided into two groups. Treatment group was given acitretin （20mg/d oral, two times a day for 1 month） and carbon dioxide laser. Control Group was given carbon diox- ide laser only, All the patients were followed for 3 months. The change of T-lymphocyte subpopulation were checked pretreatment and posttreatment. Results In the treatment group, the CD3 ＋ , CD4 ＋ and CD4 ＋/ CD8 ＋ increased but CD8 ＋ decreased after treatment. These result were significantly different between pre- treatment and posttreatment in treatment Group （P 〈 0.05 ） but not in control Group （ all P 〉 0.05 ）. When all the treatments were finished, the recurrence rate of treatment group （20.00% , 6/24 ） was significantly lower than that of control group （54.80% , 17/14） and the CD3 ＋ , CD4 ＋ , CD4 ＋/CD8 ＋ and CD8 ＋ were significantly different between the two Groups （ all P 〈 0.05 ）. Conclusions Acitretin combined with car- bon dioxide laser treatment can lower the recurrence rate. This may be contribute to the aeitretin regulating the T-lymphocyte subpopulation.
