腰骶部不典型血管淋巴样增生伴嗜酸性粒细胞增多症合并湿疹1例

摘　　要：患者男,88岁。腰骶部及臀部皮疹半年。皮肤科情况:腰骶部及右大腿外侧可见5块暗红色浸润性斑块,蚕豆至核桃大小,界清,部分斑块表面有脱屑、结痂和渗出;躯干及四肢见粟粒至绿豆大小红色斑丘疹,部分丘疹表面见结痂伴鳞屑,对称分布。皮损组织病理示:表皮细胞增生,真皮层带状血管增生,真皮浅、中层多量淋巴细胞浸润,可见少量肥大细胞、嗜酸性粒细胞及个别浆细胞。诊断:血管淋巴样增生伴嗜酸性粒细胞增多症合并湿疹。予糠酸莫米松封包治疗,现随访中。An 88-year-old male presented with skin lesions on the lumbosacral and gluteal regions for 6 months. Upon physical examination, erythematous plaques with scale, crusting and exudation were found on the lumbosa- cral and gluteal regions. The plaques were measuring from lcm to 3cm in diameter, with distincted border. Grouped small papules and macules were seen on the trunk and extremities. Histological examination showed proliferation of small blood vessels in the dermis and surrounded by a lymphoid infiltrate as well as some mastocytes and eosinophils. The patient was diagnosed as angiolymphoid hyperplasia with eosinophilia and aczema. Topical corticosteroid （mometasone furoate） was applied. The patient is still in follow-up.
