摘 要:目的对比分析局部无水乙醇注射前后兔肝超声图像与病理的改变。方法家兔20只,随机分为3组:注射无水乙醇1ml组(A组,8只),注射无水乙醇2ml组(B组,8只),不做任何处理的空白对照组(C组,4只)。超声引导下在兔肝内左侧肝叶最厚处单点单次注射规定剂量无水乙醇,在注射前后观察兔肝超声图像的改变,并在注射后第1、3、7、14天处死A、B组动物各2只,C组1只,观察肝脏大体情况,取坏死灶肝组织,观察其病理变化。结果 A、B组兔肝注射无水乙醇时在局部形成一类圆形边界尚清的回声增强区,其中B组有3只在注射时见强回声沿血管或叶间裂向外迅速扩散。13例有异常团块者肝内均可见凝固坏死灶,呈结节样硬块,色黄白或灰白,其中3例超声表现为混合回声者,坏死灶内见空洞形成;3例超声表现为回声不均匀者,肝内可见分散斑点状坏死灶。C组注射前后兔肝超声图像无改变,肝脏未见坏死灶。结论局部注射无水乙醇可对兔肝组织造成损伤,并可通过超声观察进行有效判定。Objective To discuss the comparative changes of ultrasonography and pathology on rabbit liver after percutaneous ethanol injection. Methods 20 Rabbits were randomly divided into 3 groups:ethanol injection 1 ml group(group A,n=8),ethanol injection 2 ml group(group B,n=8),control group without any treatmen(tgroup C,n=4). Required dose ethanol was injected on the thickest site of left lobe of liver once under ultrasound guidance. Ultrasound imaging changes of rabbit livers were observed before and after injection. On day 1,3,7,14 after injection,2 rabbits in group A,2 rabbits in group B and 1 rabbit in group C were killed for observation of general liver appearances and pathological changes. Results Similar round hyperechoic areas with relative clear boundaries were seen in group A and B after ethanol injection. Hyperechogenicity which spreaded outward along vessels or interlobar fissures were seen in 3 rabbits livers in group B. Coagulation necrosis areas which showed nodular lumps,yellow,or gray were seen in 13 cases with abnormal clumps under ultrasound in which 3 cases with mixed echo showed cavities in necrotic areas and 3 cases with heterogeneous echo showed scattered spotted necrosis. No ultrasound imaging changes and liver necrosis areas were seen before and after injection in group C. Conclusion Injured rabbit liver tissue can be performed by local ethanol injection of liver and observed by ultrasonography.
