大骨瓣开颅血肿清除术与颅骨钻孔血肿抽吸引流术治疗高血压脑出血的临床观察

摘　　要：目的比较大骨瓣开颅血肿清除术与颅骨钻孔血肿抽吸引流术治疗高血压脑出血的临床效果。方法将我院2008-12—2012-08收治的118例高血压脑出血患者分为2组,A组90例采用大骨瓣开颅血肿清除术治疗,B组28例采用颅骨钻孔血肿抽吸引流术治疗,对2组的治疗结果进行比较。结果 A组的血肿清除情况明显优于B组,组间差异有统计学意义(χ2=4.6221,P<0.05);B患者的日常生活能力恢复情况明显优于A组,组间差异有统计学意义(t=3.3872,P<0.05);2组无严重并发症发生。结论大骨瓣开颅血肿清除术治疗高血压脑出血的血肿清除率高,但颅骨钻孔血肿抽吸引流术治疗高血压脑出血患者日常生活能力恢复情况较佳,临床医师应根据患者情况选择适合的治疗方案。Objective To investigate the clinical effect of craniotomy hematoma evacuation with large bone flap and skull-drilling for suction and drainage of hematoma in the treatment of hypertensive cerebral hemorrhage.Methods From December 2008 to August 2012 in our hospital,118 patients with hypertensive cerebral hemorrhage were divided into group A and group B.90 patients in group A were treated by craniotomy hematoma evacuation with large bone flap,and 28 patients in group B were treated by skull-drilling for suction and drainage.The clinical effect of two groups were compared.Results The hematoma evacuation situation in the group A was significantly better than that in the group B,which had a ctatistical difference（P0.05）;The daily living skill recovery of patients in the group B was better than that in the group A,which had a statical oifference（P0.05）;There was no serious complications in two groups.Conclusion Craniotomy hematoma evacuation with large bone flap in the treatment of hypertensive cerebral hemorrhage has higher hematoma clearance rate,but skull-drilling for suction and drainage of hematoma in the treatment of hypertensive cerebral hemorrhage has better daily living skill recovery of patients.Clinicians should choose appropriate treatment according to the patient＇s condition.
