摘 要:目的通过体外实验探索以血卟啉单甲醚作为光敏剂的光动力疗法(PDT)对乳腺癌干细胞的作用及可能机制,寻找可能抑制乳腺癌干细胞的有效方法。方法采用流式细胞仪从MCF-7乳腺癌细胞系中分离出表型为CD44+CD24-/lowESA+乳腺癌干细胞,分别检测MCF-7乳腺癌干细胞及分选后的乳腺癌干细胞在空白处理组、单纯光照组、单纯光敏剂组和PDT组相对应的细胞生长抑制率,并用流式细胞仪检测其早期凋亡率。结果流式方法分选的MCF-7乳腺癌干细胞系干细胞比例为(1.2±0.4)%。空白对照组、单纯照射组、单纯光敏剂组及PDT组MCF-7乳腺癌干细胞生长抑制率比较,差异有统计学意义(P<0.05);其中PDT组较空白对照组、单纯照射组、单纯光敏剂组均升高,差异有统计学意义(P<0.05)。空白对照组、单纯照射组、单纯光敏剂组及PDT组未分选MCF-7乳腺癌干细胞生长抑制率比较,差异有统计学意义(P<0.05);其中PDT组较空白对照组、单纯照射组、单纯光敏剂组均升高,差异有统计学意义(P<0.05)。空白对照组、单纯照射组及PDT组MCF-7乳腺癌干细胞与未分选MCF-7乳腺癌干细胞生长抑制率比较,差异均有统计学意义(P<0.05)。空白对照组、单纯照射组、单纯光敏剂组与PDT组乳腺癌干细胞凋亡率比较,差异有统计学意义(P<0.05);其中PDT组较空白对照组、单纯照射组、单纯光敏剂组均升高,差异有统计学意义(P<0.05)。结论以血卟啉单甲醚作为光敏剂的PDT对乳腺癌干细胞具有一定程度的抑制作用,其抑制机制可能与诱导细胞凋亡有关,但是仍不可避免地存在耐受。因此单独使用PDT的治疗效果有限,需联合其他治疗以期达到更好的预后。Objective To verify whether photodynamic therapy(PDT) is effective on inhibiting breast cancer stem cells and its possible mechanism in vitro by choosing hematoporphyrin monomethyl(HMME) as photosensitizer of PDT in our research.Methods CD44 +CD24-/ low ESA+ cells were isolated from breast cancer cell line MCF-7 as breast cancer stem cells by using flow cytometry.Together with the MCF-7 breast cancer cells,they were divided into four groups: the control group,laser alone group,photosensitizer alone group and the PDT group.The inhibition effects were evaluated by methylthiazolyl tetrazolium(MTT) assay.Apoptosis was analyzed by using flow cytometry.Results The proportion of stem cells in breast cancer cell line MCF-7 sorted by flow cytometry was(1.2 ± 0.4) %.The inhibition rates of MCF-7 breast cancer stem cells in control group,laser alone group,photosensitizer alone group and PDT group showed statistically significant differences(P《0.05),and among them,the inhibition rate in PDT group was significantly higher than those in the other three groups(P《0.05).While the inhibition rates of unsorted MCF-7 breast cancer stem cells in control group,laser alone group,photosensitizer alone group and PDT group showed statistically significant differences(P《0.05),and among them,the inhibition rate in PDT group was significantly higher than those in the other three groups(P《0.05).The inhibition rates and apoptosis rates of the four groups of sorted MCF-7 breast cancer stem cells showed statistically significant differences compared with the four groups of unsorted MCF-7 breast cancer stem cells(P《0.05),and among them,the level of PDT group was significantly higher than those of the other three groups(P《0.05).Conclusion Photodynamic therapy shows a certain degree of inhibition on breast cancer stem cells by inducing apotosis,but it also has tolerance.Photodynamic therapy alone has limited effect.To achieve better prognosis,combined therapy is necessary.
