摘 要:目的探讨双向转诊最优激励机制,为双向转诊激励机制的落实提供决策依据。方法分析激励双向转诊的主要方案与措施,建立多层次的双向转诊激励机制评价模型,利用专家判断法和层次分析法确定权重值,并进行一致性检验。结果构建了双向转诊最优激励机制层次结构,激励对象为患者、社康医生、医院医生的权重值分别为0.2583、0.1047、0.6370;激励方案为医保奖惩、社康建设、转诊平台、宣传交流的权重值分别为0.4950、0.3103、0.1164、0.0783。结论实施双向转诊激励,排在首位的激励对象是医院医生,其次是患者;最有效的激励方案是医保奖惩,其次是社康建设。Objective To explore the optimal incentive mechanism of two - way referral so as to provide an evident for making decision in implementation of two - way referral incentive mechanism. Methods By analyzing the main schemes and measures a multi - hierarchy evaluation model of incentive mechanism of two - way referral was set up. And then the weight val- ues were determined by using expert judgment and analytic hierarchy process, and consistency check was conducted. Results The multi - hierarchical structure was set up. The weight values of patients, community doctors and hospital doctors as the incen- tive subjects were respectively 0. 2583, 0. 1047, and 0. 6370. The weight values of healthcare reward and punishment, con- struction of community medical center, referral platform, and propaganda -communication as the incentive schemes were respectively 0. 4950, 0. 3103, 0. 1164, and 0. 0783. Conclusion In implementation of two - way referral incentive, the first incentive subjects are hospital doctors, followed by patients; and the most effective incentive scheme is " healthcare reward and pun- ishment" , followed by " construction of community medical center".
