摘 要:目的对超声血管增强技术(VET)在胎儿肺静脉检查中的应用价值进行探析和评价。方法选取2011年7月~2012年11月在我院行产前超声检查的孕妇368例,根据孕龄将368例胎儿肺静脉分为Ⅰ组(14~21周,104例)、Ⅱ组(22~29周,213例)和Ⅲ组(30~37周,51例);对所有孕妇均进行四种方式的肺静脉检查,包括二维超声心电图(2-DE)、彩色多普勒血流显像(CDFI)、彩色多普勒能量图(CDE)和VET,对检查结果进行对比分析,同时比较四种方法在不同孕龄组中的应用效果。结果Ⅰ组104例中2-DE、CDFI、CDE和VET分别显示17例(16.3%)、35例(33.7%)、46例(44.2%)和87例(83.7%);Ⅱ组213例中2-DE、CDFI、CDE和VET分别显示46例(21.6%)、99例(45.8%)、112例(52.6%)和197例(92.5%);Ⅲ组51例中2-DE、CDFI、CDE和VET分别显示16例(31.4%)、31例(60.1%)、30例(58.8%)和32例(62.7%)。四种检查方式中VET显示率最高,且VET检查Ⅱ组显示率明显增加,差异具有统计学意义(P<0.05)。结论 VET在胎儿肺静脉的检查中具有更高的显示率,且结果表明胎儿肺静脉检查的最佳时间是中孕期。为胎儿肺静脉临床检查提供了一些可靠依据。Objective To analyze and evaluate the application value of ultrasound vascular enhancement technology(VET) in fetal pulmonary vein inspection. Methods A total of 368 pregnant women done prenatal ultrasound examina-tion in our hospital from July 2011 to November 2012 were choosed. The fetal pulmonary vein of 368 cases were di-vided into group I (14-21 weeks, 104 cases), group II (22-29 weeks, 213 cases) and group Dt (30-37 weeks,51 cases)according to gestational age. All pregnant women were done four kinds of pulmonary vein inspection, including two-dimensional echocardiogram(2-DE), color doppler flow imaging(CDFI) and color doppler energy (CDE) and VET. Theresults of four kinds of inspection methods were compared and analyzed, and the application result of four kinds ofmethods in different gestational age groups were compared. Results In 104 cases of group I,the number or propor-tion of patients having the display in 2-DE, CDFI, CDE and VET were 17 cases (16.3%), 35 cases (33.7%), 46 cases(44.2%) and 87 cases(83.7%) respectively; In 213 cases of group II , the number or proportion were 46 cases(21.6%),99 cases (45.8%), 112 cases (52.6%) and 197 cases (92.5%) respectively; In 51 cases of group III, the number or pro-portion were 16 cases (31.4%), 31 cases (60.1%), 30 cases (58.8%) and 32 cases(62.7%) respectively. The display rateof VET was the highest among four kinds of inspection methods,and the display rate of VET in group II increasedmarkedly, the difference was statistically significant (P 〈 0.05). Conclusion VET has higher display rate in fetal pul-monary vein inspection, and the results show that the best time to inspect fetal pulmonary vein is second trimester. VET can provide some reliable basis for clinical fetal pulmonary vein inspection.
