摘  要:目的探讨肾上腺髓质素(ADM)在卵巢上皮性肿瘤组织中的表达及其与上皮性卵巢癌的临床病理特征和血管生成的关系。方法采用ADM多克隆抗体免疫组织化学染色法检测83例上皮性卵巢肿瘤(恶性66例、良性17例)患者ADM的表达情况,以CD34单克隆抗体标记血管内皮细胞,观察83例肿瘤组织微血管密度(MVD)。结果恶性上皮性卵巢肿瘤组ADM的阳性表达率高于良性上皮性卵巢肿瘤组(69.70%vs 41.18%)(P〈0.05)。在上皮性卵巢癌中,组织学分级3级的ADM阳性表达率高于1~2级(95.24%vs 57.78%)(P〈0.05)。有淋巴结转移组ADM的阳性表达率高于无淋巴结转移组(90.00%vs 57.78%)(P〈0.05)。ADM的表达与上皮性卵巢癌患者的年龄、疾病的临床分期、组织学类型无显著相关性(P〉0.05)。上皮性卵巢癌中MVD值为(34.30±21.47)个/400×,显著高于良性卵巢肿瘤组织中MVD值(16.35±8.84)个/400×(P〈0.05)。上皮性卵巢癌中ADM的表达与MVD无相关性(P〉0.05)。结论 ADM可能在上皮性卵巢癌的发生、发展中起作用,该作用可能并非通过促进血管生成而实现。ADM与上皮性卵巢癌的恶性程度及不良预后有关,对其检测将有助于卵巢癌早期高危患者的识别、恶性程度的判断及预后评估。Objective To study the expression of adrenomedullin(ADM) in epithelial ovarian tumors and the relationship between ADM and clinical-pathological features and angiogenesis in epithelial ovarian cancers.Methods The expression of ADM was detected by immunohistochemistry in 66 cases of epithelial ovarian cancers and 17 cases of epithelial ovarian benign tumors.Microvessel density(MVD) in 83 cases of epithelial ovarian tumors were observed by immunohischemistry with monoclonal antibody CD34 to mark vascular endotheliocytes.Results The positive rates of ADM expression in malignant and benign epithelial ovarian tumors were 69.70% and 41.18%.The former was significant higher than the latter(P 0.05).The positive rate(95.24%) of ADM expression in the cases with histological grade 3 was significant higher than that(57.78%) in the cases with histological grade 1-2(P 0.05).The positive rate(90.00%) of ADM expression in the cases with lymph node metastasis was significant higher than that(57.78%) in the cases without lymph node metastasis(P 0.05).In epithelial ovarian cancers,the associations between the expression of ADM and the patients' age,clinical stage and histological subtype were not statistically significant(P 0.05).MVD was(34.30 ± 21.47) /400 × in the malignant tumors and(16.35 ± 8.84) /400 × in the benign tumors.The former was significant higher than the latter(P 0.05).Spearman correlation between the expression of ADM and MVD was not statistically significant(P 0.05).Conclusion ADM might be involved in the genesis and progress of epithelial ovarian cancer and it might not work by its angiogenesis.ADM is related to malignant degree and poor prognosis.Detecting ADM expression level can help to identify high-risk patients in early stage ovarian cancer,evaculate patients' prognosis and judge malignant degree.
