摘 要:目的探讨伤后早期应用乌司他丁干预对海水浸泡兔腹部开放伤炎性介质、血流动力学及电解质的影响。方法将12只新西兰雄性大白兔随机分为实验组和对照组,每组6只,两组均予建立腹部开放伤模型后用海水浸泡60 min,然后对照组给予平衡盐液静脉滴注,实验组给予乌司他丁50 000 U/kg加入0.9%氯化钠注射液静脉滴注,并分别于人工海水浸泡前、浸泡60 min及用药60 min时检测两组外周血肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、IL-2、IL-8、平均动脉压(MAP)、中心静脉压(CVP)及钠离子(Na+)、钾离子(K+)、氯离子(Cl-)浓度的变化。结果浸泡60 min后,两组外周血TNF-α、IL-1β、IL-2、IL-8、Na+、K+、Cl-均较浸泡前升高,而MAP、CVP则降低(P<0.05);用药后60 min,实验组外周血TNF-α、IL-1β、IL-2、IL-8和Na+、K+、Cl-均低于对照组,但与浸泡60 min比较差异无统计学意义(P>0.05),MAP、CVP均高于对照组(P<0.05),对照组外周血TNF-α、IL-1β、IL-2、IL-8和Na+、K+、Cl-高于浸泡60 min,MAP低于浸泡60 min(P<0.05,P<0.01)。结论乌司他丁能显著降低海水浸泡腹部开放伤炎性介质的过度释放,显著改善血流动力学及血电解质紊乱,可能能够阻断全身炎性反应综合征的病理过程。Objective To evaluate the effect of Ulinastatin(UTI) intervention on inflammatory reaction,hemodynamic parameters and electrolyte for open celiac injury complicated with seawater immersion during prophase in rabbits.Methods A total of 12 New Zealand male rabbits were randomly divided into control group(n = 6) and experiment group(UTI group,n = 6).The two groups were established models of open celiac injury with seawater immersion for 60 min.The control group was treated with intravenously guttae of balance salt solution,while experiment group was treated with intravenously guttae by 50 000 U /kg of Ulinastatin into physiological saline.Changes of cony peripheral blood of tumor necrosis factor(TNF-α),interleukin-1β(IL-1β),interleukin-2(IL-2),interleukin-8(IL-8),mean arterial blood pressure(MAP),central venous pressure(CVP) and concentrations of Na+,K+ and Cl-in the two groups were observed before soaking,at 60 min of soaking and after medication for 60 min.Results After 60 min of soaking,peripheral blood TNF-α,IL-1β,IL-2,IL-8,Na+,K+ and Cl-in two groups were significantly increased than those before soaking,while MAP and CVP levels were significantly decreased(P 0.05);after 60 min of medication,concentrations of TNF-α,IL-1β,IL-2 and IL-8,and Na+,K+ and Cl-in experiment group were significantly lower than those in control group,but the difference between those after 60 min of medication with those after 60 min of soaking was no statistical significance(P 0.05),while MAP and CVP levels were significantly higher than those in control group(P 0.05);peripheral blood TNF-α,IL-1β,IL-2,IL-8,Na+,K+ and Cl-in control group were higher than those at 60 min of soaking,while MAP level was significantly lower than that at 60 min of soaking(P 0.05,P 0.01).Conclusion UTI may reduce the overmuch release of inflammatory mediators in open celiac injury complicated with seawater immersion,and improve hemodynamic parameters and plasma electrolyte disturbances
