摘  要:目的探讨羟基磷灰石义眼台肌锥内自体巩膜后植入术的临床疗效。方法回顾分析我院自2008年1月~2012年8月以来行羟基磷灰石义眼台肌锥内自体巩膜后植入术78例患者,临床资料。结果本组患者术后随访3~36个月,所有患者结膜切口愈合好,术后炎症反应轻,义眼台活动良好,位置居中,眼窝无凹陷,双眼对称,义眼片可轻微活动,外观满意,无义眼台外露脱出,无眶内颅内感染、上睑下垂、眼睑外翻及结膜囊狭窄等并发症。3例出现植入性囊肿,2例出现结膜肉芽组织增生,1例出现上睑凹陷。结论羟基磷灰石义眼台肌锥内自体巩膜后植入术对眼外肌及眶脂肪的损伤较轻,并发症少、恢复快,术后眼球活动度好,眼眶饱满,眼窝无凹陷,双眼对称,外观自然逼真,是一种值得临床推广的安全有效的手术方式。Objective To observe the clinical therapeutic effects of hydroxylapatite implantation in the muscle cone covered with autologous sclera.Methods A retrospective study was conducted to review the files of 78 eyes(50 males,28 females) with hydroxylapatite implantation in the muscle cone covered with autologous sclera from January 2008 to August 2012 in our hospital.Results The time of postoperative follow-up ranged from 3 to 36 months.the conjunctival incisions of all patients were healing well,the postoperative inflammatory reaction was light,and the activities and position of orbital implants were very good,eye socket was non-pitting,both eyes were symmetrical,appearance was satisfactory.Complications such as exposition of orbital implant,conjunctival sac narrow,infection or extrusion did not occur in any case.three patients of cyst of conjunctiva in the postoperative scar area were encountered,and 2 cases had conjunctival granulation tissue hyperplasia,sunken upper eyelid occurred in 1 cases.Conclusion Hydroxyapatite implantation in the muscle cone covered with autologous sclera has several advantages,including light damages to extraocular muscle and orbital fat,less complications and rapid and efficient rehabilitation,good postoperative eye movements and natural appearance.This technique is useful,and it is valuable application extensively.
