甲硝唑联合康复新液全结肠灌洗在Ⅰ期肿瘤切除肠吻合术中的应用

摘　　要：目的探讨Ⅰ期肿瘤切除肠吻合术联合术中全结肠灌洗治疗左半结肠癌并梗阻患者的临床效果。方法选择我院2007年5月至2012年5月收治的90例左半结肠癌并肠梗阻患者为研究对象,应用随机数字表法将其分为对照组和实验组各45例,两组患者均给予Ⅰ期肿瘤切除肠吻合术联合术中全结肠灌洗治疗,对照组患者术中全结肠灌洗治疗时采用甲硝唑液,实验组患者术中全结肠灌洗治疗时采用甲硝唑+康复新液,比较两组患者临床疗效和吻合口瘘、腹腔感染、切口感染、肺部感染等并发症的发生情况。结果实验组患者总有效率为100.00%,对照组为97.78%,差异无统计学意义(P>0.05),实验组患者并发症的发生率(8.89%)明显低于对照组(26.67%),差异有统计学意义(P<0.05)。结论Ⅰ期肿瘤切除肠吻合术联合术中全结肠灌洗(甲硝唑+康复新液)治疗左半结肠癌并梗阻临床疗效确切,安全高效,并发症少,值得临床推广。Objective To investigate the clinical efficacy of Ⅰ period tumor resection intestinal anastomosis combined with whole colon lavage in the treatment of left colon cancer patients with obstruction. Methods Ninety left colon cancer patients with obstruction who were treated in our hospital from May 2007 to May 2012 were collect- ed. The patients were divided into the control group （n=45） and the observation group （n=45） by random number ta- ble. All the patients were given I period tumor resection intestinal anastomosis combined with whole colon lavage, but the patients in the control group were given whole colon lavage during the operation by metronidazole lotion and those in the observation group were given metronidazole lotion＋Kangfuxin solution. The clinical efficacy and adversedrug reactions of two groups were evaluated, including anastomotic leakage, intra-abdominal infection, wound infec- tion, lung infection. Results The clinical efficacy of the two groups showed no statistically significant difference （100.00% in the observation group and 97.78% in the control group,P〉0.05）. The adverse drug reaction rate of the ob- servation group was lower than that in the control group （8.89% vs 26.67%, P〈0.05）. Conclusion The clinical effica- cy of treating colon cancer patients with obstruction by Ⅰ period tumor resection intestinal anastomosis combined with whole colon lavage during the operation （metronidazole lotion＋Kangfuxin solution） is significant, with high effi- cacy, few side effects, and high biological safety, which is worthy of clinical application.
