摘 要:目的:探讨MMP-12启动子区-82bp A/G单核苷酸多态性(SNP)与我国北方人非小细胞肺癌(NSCLC)遗传易感性的关系。方法:采用基于医院的病例-对照研究方法,收集300例非小细胞肺癌患者(其中鳞癌161例,腺癌139例)和300例健康对照个体的静脉抗凝血5 ml,以蛋白酶K消化-饱和氯化钠盐析法提取外周血白细胞DNA,采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法对基质金属蛋白酶MMP-12-82A/G单核苷酸多态性进行基因分型,比较非小细胞肺癌病例组与健康对照组之间等位基因及基因型分布。结果:非小细胞肺癌病例组中吸烟个体的比例(62.0%)明显高于健康对照组(36.3%),P<0.01,经年龄、性别校正后的OR值为3.74(95%CI=2.451~5.705)。健康对照组中的MMP-12基因启动子区转录起始点上游82 bp处A/G SNP的基因型分布符合Hardy-Weinberg平衡(P>0.05)。在病例组和健康对照组之间及根据吸烟状况和病理类型分层分析MMP-12基因-82A/G SNP基因型和等位基因频率分布均无显著性差异(P>0.05)。结论:吸烟可以显著增加非小细胞肺癌的发病风险。MMP-12基因-82A/G SNP与非小细胞肺癌的发病风险无关。Objective:To investigate the association between matrix metalloproteinases-12(MMP-12-82A/G SNP) polymorphism and susceptibiliy of non-small cell lung cancer(NSCLC) in northern China.Methods: This hospital-based case-control study included 300 patients with non-small cell lung cancer(161 squamous carcinoma and 139 adenocarcinoma) and 300 healthy controls.Total DNA was extracted form peripheral blood leucocyte using proteinase K digestion followed by a salting out procedure.Polymorphisms of MMP-12 gene were analyzed by PCR-restriction fragment length polymorphism(RFLP).Results: The percentage of smokers in the patients(62.0%) was significantly higher than that in healthy controls(36.3%,P0.01).Smoking may increase the risk of NSCLC(age and gender adjusted OR=3.74,95% CI=2.451-5.705).The distribution of MMP-12-82 A/G SNP in the promoter region of healthy controls was not significantly different from the patients with NSCLC evaluated by Hardy-Weinberg equilibrium(P0.05).The allelotype and genotype distribution of the MMP-12-82A/G SNP was not significantly different between NSCLC patients and healthy controls according to smoking status and histological type(P0.05).Conclusion: Smoking increases the risk of developing NSCLC.The allelotype and genotype distribution of MMP-12-82A/G SNP in the NSCLC patients is not significantly different from that in healthy controls.
