含呋喃唑酮和左氧氟沙星四联疗法根除幽门螺杆菌的疗效观察

摘　　要：目的评价含呋喃唑酮和左氧氟沙星的四联疗法作为初始方案根除幽门螺杆菌(H.pylori)的疗效和安全性。方法选择符合条件的138例H.pylori阳性患者,随机分为两组。治疗组采用雷贝拉唑(10mg,每日2次)、呋喃唑酮(100mg,每日2次)、左氧氟沙星(500mg,每日1次)和胶体果胶铋(200mg,每日2次)组成的四联疗法;对照组采用雷贝拉唑(10mg,每日2次)、阿莫西林(1 000mg,每日2次)、克拉霉素(500mg,每日2次)和胶体果胶铋(200mg,每日2次)组成的四联疗法。两组疗程均为14天。治疗结束4周后行13 C-尿素呼气试验,判断H.pylori的根除情况,同时观察治疗过程中的不良反应。结果治疗组和对照组H.pylori根除率意向性分析(ITT)和符合方案数据分析(PP)分别为91.3%(63/69)vs 75.4%(52/69),92.6%(63/68)vs 77.6%(52/67)(均P<0.05)。两组均未见明显不良反应。结论含呋喃唑酮和左氧氟沙星四联疗法H.pylori根除率高,可以考虑作为初次治疗的方案选择。Objective To appraise the efficacy and safety of the furazolidone and levofloxacin-containing quadruple therapy as first-line therapy for the eradication of Helicobacter pylori （H. pylori）. Methods A total of 138 patients with H. pylori infections who met the requirements were randomized to treatment group and control group. The treatment group was administered with the quadruple therapy consisting of rabeprazolel（10 rag, twice daily）, furazolidone（100 mg, twice daily）, levofloxacin （500 mg, twice daily） and colloidal bismuth pectin （200 mg, twice daily）. The control group was administered rabeprazolel （10 mg, twice daily）, amoxicillin（1 000 rag, twice daily）, clarithromycin（500 rag,twice daily） and colloidal bismuth pectin （200 rag,twice daily）. The treatment lasted 14 days in both groups. H. pylori eradication was assessed by 13 C-urea breath test four weeks after cessation of therapy,and the safety was also evaluated. Reshlts The eradication rates of intention-to-treat （ITT） and per-protocol（PP） in treatment group and control group were as follows：91.3%（63/69） vs 75.4%（52/69）,92.6% （63/68） vs 77.6%（52/67） （both P 0.05）. No serious adverse effects were found in two groups. Conclusion The furazolidone and levofloxacin-containing quadruple therapy can achieve a higher eradication rate,and it may be one of the first choices for H. pylori eradication.
