摘  要:目的研究视网膜色素变性(RP)的多焦视网膜电图(ERG)特性和评价多焦ERG应用价值。方法检测了 12例(24眼)RP患者的多焦 ERG,测试野为26.6°×22.1°,记录 103个部位的反应,同时检测全视野 ERG。结果RP眼在各个环的a波和b波潜伏期、b波振幅密度和振幅之和均较正常值有显著延长和降低(P<0.05或P<0.01)。50%患眼在第1环可记录到潜伏期正常、振幅密度和总和均降低的b波,而75%患眼的全视野ERG呈平坦反应。结论 多焦ERG能更有效地评价RP的黄斑区功能。Objective To investigate the characteristics of the multifocal electroretinogram (ERG) and to evaluate the application values of the multifocal ERG in retinitis pigmentosa (RP). Methods Multifocal ERGs were obtained using VERIS 4.0 visual evoked response imaging system for 12 patients (22 eyes) with RP and 36 normal subjects (42 eyes). The stimulative visual angles subtended ±26.6° horizontally and ±22.1° vertically. Burian-Allen contact lens electrode was used to record the responses from 103 retinal locations in 8 min (16 segments). The full field ERGs were obtained in the same patients. Results The latencies of a wave and b wave delayed significantly, and the amplitude densities and the amplitude summary of b wave decreased markedly at all rings in RP group compared with normal group (P<0. 05 or P<0.01 ). Fifty percent affected eyes at ring 1 and 33. 3% affected eyes at ring 2 showed normal latencies and reduced amplitude densities and amplitude summary of b wave of multifocal ERG. 92% affected eyes showed delayed latency and reduced amplitude densities and amplitude summary of b wave at ring 5 and ring 6 of MERG. Only 25% affected eyes recorded the markedly reduced amplitude and delayed latency of full field ERG,while 75% affected eyes showed flat response of full field ERG. conclusion Multifocal ERG can provide more information and evaluate the macular function more effectively than full field ERG for RP.
