摘 要:目的探讨矫正水平斜视的安全有效方法。方法在直肌边缘切开术基础上,自肌附着点两端以10°~20°角斜形向中央剪开肌腱约3mm,保留中央部肌腱约3~4mm。采用改良直肌边缘切开术矫正水平斜视16例,并与直肌后退术15例相对照。结果改良直肌边缘切开术与直肌后退术矫正效果相似。结论改良直肌边缘切开术操作简单,安全有效,可避免直肌边缘切开术可能引起的眼位垂直偏斜,并可避免直肌后退术可能引起的巩膜穿孔等危险。该术式可替代直肌后退术与直肌边缘切开术。Objective To investigate the method of correction of horizontal squint. Methods A 3mm muscular tendon was incised by 10- 20 degree angle of oblique at hibateral of muscular attachment, and a 3-4mm central muscular tendon was preserved. The improved marginal myotomy was used to correct 16 cases with horizontal concomitant squint, and was compared with 15 cases of muscular recession. Results The effect of improved marginal myotomy was similar to that of muscular recession. The entopic or basic entopic success probability in improved marginal myotomy was 72. 7% in external squint, and 80% in control group. There was no significant difference (P>0. 05). The entopic or basic entopic success rate in improved marginal myotomy was 80% in esotropia and 77. 8% in control group. Conclusion The improved marginal myotomy is a safe, effective and simple method instead of marginal myotomy and muscular recession. It can prevent vertical diviation following marginal myotomy and penetrating ocular trauma following muscular recession.
