臂丛神经损伤后康复治疗方案的探究

摘　　要：目的观察规范化康复治疗方案在臂丛神经损伤患者中的效果。方法选择2011年1月～2012年12月就诊的50例臂丛神经损伤患者,分为治疗组和对照组,每组各25例;治疗组给予规范化的康复治疗(包括低频脉冲电刺激,中频电疗、运动疗法、中医推拿治疗、心理辅导),对照组给予一般治疗(即心理辅导和患者在家中自己进行神经肌肉电刺激、体疗);比较两组患者臂丛功能评分和神经电位恢复情况进行。结果①治疗前两组患者的全臂丛损伤、上干损伤及束支部损伤患者臂丛功能评估分值差异无统计学意义(P>0.05);治疗后治疗组全臂丛损伤、上干损伤及束支部损伤患者臂丛功能评估分值均高于对照组,差异均有统计学意义(均P<0.05),且治疗组治疗后全臂丛损伤、上干损伤及束支部损伤患者臂丛功能评估分值均高于治疗前,差异均有统计学意义(均P<0.05)。②治疗组治疗后恢复率为76%(19/25),对照组恢复率为44%(11/25),两组差异有统计学意义(P<0.05)。结论臂丛神经损伤后的患者经过规范化康复治疗,可以很好地促进患者的臂丛神经功能的再生及功能的恢复,临床上应该大力推广。Objective To observe the effect of standardized rehabilitation in treatment of patients with brachial plexus injury.Methods 50 patients with brachial plexus injury from January 2011 to December 2012 were selected and di vided into treatment group and control group with 25 cases in each group.The treatment group were treated with tan dardized rehabilitation,which included low frequency pulsed electric stimulation,medium frequency electrotherapy,exercise therapy,tuina therapy and psychotherapy.The control group were treated with home-in low frequency pulsed electric stimulation,psychotherapy and motor function training.Regeneration score of the nerve and recovery state of nerve potential in the two groups were compared.Results ①Total brachial plexus injury,superior trunk injury,bundle branch lesions functional assessment scores in the two groups before the treatment was found had no statistically sig nificant difference（P 0.05）;total brachial plexus injury,superior trunk injury,bundle branch lesions functional as sessment scores in treatment group after the treatment were higher than those in control group,the differences were statistically significant（P 0.05）;total brachial plexus injury,superior trunk injury,bundle branch lesions functional assessment scores in treatment group after the treatment were higher than those before the treatment,the differences were statistically significant（P 0.05）.②Recovery rate in treatment group after the treatment was 76%（19/25）,re covery rate in control group after the treatment was 44%（11/25）,the differences were statistically significant（P 0.05）.Conclusion Standardized rehabilitation is effective in the regeneration of the nerve and the function improving of patients with brachial plexus injury.It is worthy of clinical promotion.
