老年人胰腺体尾部癌26例临床分析

摘　　要：目的 探讨老年人胰腺体尾部癌的临床规律，提高早期诊断率。方法 对我院１９９０年６月至１９９９年６月收治的２６例老年胰腺体尾部癌病人的临床资料进行回顾性分析。结果 老年人胰腺体尾部癌临床症状、体征不典型，有典型腹痛、腹部包块者仅占２５％。其误诊率高达５３．８％；并发症以梗阻性黄疸、胰源性腹泻、肠梗阻等为多见，其发生率为５０％。Ｂ型超声波、腹部ＣＴ或ＭＲＩ检查是诊断胰腺体尾部癌的主要手段。外科手术切除是治疗老年胰腺体尾部癌的主要措施之一。结论 提高对本病的认识，早期诊断、早期治疗是降低病死率、改善预后的关键。Objective To discover the clinical characteristics of carcinoma of the body and tail of the pancreas in elderly patients and improve the diagnostic accurate rate of carcinoma of the body and tail of the pancreas in early stage. Methods Clinical data of 26 elderly patients with carcinoma of the body and tall in the pancreas from June,1990 to June, 1999 were retrospectively analysed. Results Symptoms and signs of carcinoma of the body and tail in the pancreas in the elderly patients were atypical. Typical clinical manifestations including abdominal pain and abdominal mass occured just in 25% of the elderly patients suffering from carcinoma of the body and tail of the pancreas. The misdiagnostic rate of carcinoma of the body and tail of the pancreas in the elderly patients was as high as 53.8% and the complications of carcinoma of the body and tailof the pancreas including obstructive jaundice,pancreatic diarrhea and alimentary tract obstrUction occured frequently(50%).Type B ultrasound wave examination,computed tomography(CT) or magnetic resonance imaging(MRI) were of singificance in diagosis of carcinoma of the body and tail of the pancreas. Resection of pancreatic mass was one of main ways in treating elderly patients wth carcinoma of the body and tail in the pancreas. Conclusion To increase the knowledge of carcinoma of the body and tail in the pancreas and early diagnose and treatment are key in decreasing mortality and improving prognosis of this disease.
