摘  要:目的研究粒细胞集落刺激因子(G-CSF)对大鼠脑出血(ICH)后脑水肿及胶质纤维酸性蛋白(GFAP)的影响,探讨G-CSF在ICH中的神经保护作用及可能机制。方法健康雄性SD大鼠随机分为假手术组、模型组、治疗组。断尾取自体血法制备大鼠ICH模型,治疗组于造模1 h后腹腔注射人重组粒细胞集落刺激因子(rhG-CSF)60μg/kg。3组大鼠于术后6、24、48、72 h及7、10 d处死,干湿重法测定脑组织含水量,免疫组化检测血肿周围的基质金属蛋白酶9(MMP-9)、GFAP阳性细胞。结果模型组脑组织含水量及MMP-9高于假手术组(P均<0.05),造模后48、72 h最明显,且与组内6、24 h及7、10 d相比差异有统计学意义(P均<0.05);治疗组脑组织含水量及MMP-9低于模型组(P均<0.05)。假手术组脑组织含水量与MMP-9表达无相关性(r=0.429,P>0.05),模型组及治疗组脑组织含水量与MMP-9表达呈正相关(r分别为0.922、0.844,P均<0.05)。假手术组未见GFAP阳性细胞表达;模型组6 h出现少量GFAP阳性细胞表达,48 h开始增多,72 h有大量表达,7 d达高峰;治疗组GFAP阳性细胞表达6 h与模型组相似,24、48、72 h及7、10 d较模型组减少(P均<0.05)。结论 ICH后G-CSF可抑制MMP-9表达,减轻脑水肿程度,抑制星形胶质细胞过度活化,对出血后脑组织具有保护作用。Objective To explore the influence of granulocyte colony-stimulating factor(G-CSF) on the brain edema and glial fibrillary acidic protein(GFAP) in intracerebral hemorrhage rats and to investigate its neuroprotective effect and mechanism.Methods Healthy and male SD rats were randomly divided into three groups: the sham operation group,the model group and the treatment group.The model of ICH was made by cutting off tail of rat to obtain blood.The treatment group was administered rhG-CSF(60 μg / kg) by intraperitoneal injection 1 h after operation.The brain edema was measured by dry-wet weigh method,MMP-9 and GFAP positive cells were measured by immunohistochemical method in every group after the operation 6 h,24 h,48 h,72 h,7 d and 10 d.Results The brain edema and contents of MMP-9 positive cells in the model group were markedly higher than the sham operation group(all P 0.05),reaching the peak at 48 h,72 h and the difference was significant compared with those at 6 h,24 h,7 d and 10 d(all P 0.05);the brain edema and contents of MMP-9 positive cells in the treatment group were significantly lower than the model group(all P 0.05).In the sham operation group,the changes of the contents of MMP-9 positive cells were not correlated with the brain edema(r =0.429,P 0.05).In the model and treatment groups,the changes of the contents of MMP-9 positive cells were positively correlated with the brain edema(r = 0.922,0.844,respectively;both P 0.05).There was not GFAP-positive cell in the sham operation group;in the model group,the GFAP was little expressed at 6 h,increased at 48 h,and there were large numbers of GFAP-positive cells at 72 h then reached a peak at 7 d;the expression of GFAP in the treatment group was identical to that of the model group at 6 h,however,the number of GFAP-positive cells was obvious decreased compared with that in the model group at 24 h,48 h,72 h and 7 d after intervention(all P 0.05).Conclusion G-CSF can down-regulate MMP-9 expression,decrease the brain edem
