摘 要:目的检测N0期声门上型喉鳞状细胞癌(squamous cell carcinoma,SCC)颈部淋巴结中肿瘤转移抑制基因KAI1/CD82的表达,并分析其与临床组织病理学等相关因素的关系,以进一步揭示KAI1/CD82在声门上型喉SCC颈淋巴结隐匿性转移发生、发展中的作用及临床意义。方法采用免疫组化链霉素抗生物素蛋白-过氧化物酶法,检测60例N0期声门上型喉SCC颈清扫淋巴结中KAI1/CD82的表达、12例新鲜喉癌组织和同期距癌周至少0.5 cm的正常喉黏膜组织KAI1/CD82的表达,以及KAI1/CD82的表达水平与声门上型喉SCC颈淋巴结隐匿性转移、病理分化类型及浸润程度等临床病理因素的关系。结果 60例N0期声门上型喉SCC患者颈清扫淋巴结327个,进行常规病理检查和KAI1/CD82的免疫组化检测发现84个淋巴结KAI1/CD82表达降低。其中,常规病理检查发现46个淋巴结有转移者,KAI1/CD82检测表达也降低;38个淋巴结常规病理检查未发现转移者,KAI1/CD82表达也降低。检测出KAI1/CD82在N0期声门上型喉SCC有颈淋巴结隐匿性转移组的阳性表达率明显低于无颈淋巴结隐匿性转移组的阳性表达率(P<0.01)。KAI1/CD82在原发灶中的阳性表达显著降低。KAI1/CD82的表达与有无淋巴结隐匿性转移有关,与患者的病理分化类型、浸润程度、原发肿瘤局部淋巴结远处转移(tumor lymph node metastasis,TNM)分期、肿瘤大小有关,差异有统计学意义(P<0.05)。结论 KAI1/CD82表达与N0期声门上型喉SCC的病理分化类型有关,恶性程度越高的肿瘤KAI1/CD82表达越低;与浸润程度有关,随着T分期的增高,KAI1/CD82的表达随之降低;KAI1/CD82表达降低可能与N0期声门上型喉SCC颈淋巴结隐匿性转移有关。KAI1/CD82在N0期声门上型喉SCC的转移过程中具有一定作用,可作为预测肿瘤转移潜能的指标。ObjectiveTo detect the expression of KAI1/CD82 in N0 stage supraglottic laryngeal squamous cell carcinoma around the lymph node metastasis, and to analysis the relationship between it and their clinicopathological features. Further to reveal the affect and significance of KAI1/CD82 in association with the occurrence and development of occult metastasis. MethodsKAI1/ CD82 expression in 60 N0 stage supraglottic laryngeal carcinoma neck dissection of lymph node tissues were studied using immunohistochemical staining. The samples of tissue cutting from the distance 05 cm edge of carcinoma of larynx and 12 cases of fresh laryngeal cancer tissues (control group) were also studied, and the results were compared. The relations between the expression level of KAI1/CD82 and clinicopathological factors were evaluated by χ2 test. ResultsThe number of cervical lymph node dissection was 327 in 60 patients with supraglottic laryngeal squamous cell carcinoma. Routine pathological examination and KAI1/CD82 protein immunohistochemical detection. The expression of KAI1/CD82 in N0 Stage supraglottic laryngeal carcinoma occult cervical lymph node metastasis was lower than that in the laryngeal cancers without lymph node metastasis(P〈001). The positive expression of KAI1/CD82 in primary lesions was lower significantly. The expression of KAI1/CD82 was closely correlated with lymph node occult metastasis. It was correlative to the type of pathological differentiation, depth of invasion, TNM stage and tumor size. They were significant differences(P〈005). ConclusionKAI1/CD82 protein expression is related to the type of pathological differentiation of N0 supraglottic laryngeal carcinoma which was correlative to the depth of invasion, and TNM stage. With the increase in T staging, the KAI1/CD82 protein expression decreased. The down expression decreased of KAI1/CD82 were probably correlated with lymph node occult metastasis. KAI1/CD82 protein is considered as a target which can predict metastatic potential of tumor
