血钾、C反应蛋白和乳酸与急性百草枯中毒患者预后相关性的研究

摘　　要：目的:探讨血液生化、血气指标与急性百草枯中毒患者预后的相关性。方法:回顾性研究2002-2012年共179例急性百草枯中毒患者,按预后分为生存组和死亡组,比较中毒早期血液生化和血气各指标的水平,并建立ROC曲线观察各指标与急性百草枯中毒患者预后的相关性。结果:血钾、CRP和动脉血乳酸与患者死亡率下的ROC曲线下面积明显大于其它指标,且三者指标水平在两组患者差异有统计学意义(P<0.01),血钾、CRP和乳酸ROC曲线下的面积分别为0.886、0.810和0.856,对早期患者预后有较高分辨力,根据ROC曲线确定的最佳截断值,血钾、CRP和乳酸预测死亡率的灵敏度、特异度和Youden指数分别为0.5～0.741,0.105～1,0.5～0.646。结论:早期血钾、CRP和动脉血乳酸水平与急性百草枯中毒预后有较好的相关性。Objective：To investigate whether the levels of blood biochemistry and blood gas are associated with the prognosis of patients with acute paraquat intoxication.Method：179cases of acute paraquat poisoning patients were divided into survival and death group according to prognosis.The early blood biochemistry and blood gas level were compared and the ROC curves were established to observe the correlations between markers and the prognosis of patients with acute paraquat intoxication.Result：Potassium,CRP and lactic acid level in artery of patients in two groups have obvious difference（P0.01）.The area of potassium,CRP and lactic acid ROC curves are 0.886,0.810and 0.856according to the best truncation value of the ROC curves.The mortality sensitivity,specific degrees and Youden index predicted by potassium,CRP and lactic acid are 0.5～0.741,0.105～1,0.5～0.646respectively.Conclusion：Early potassium,CRP and lactic acid level in arterial have good correlations with prognosis of acute paraquat intoxication.
