摘  要:目的观察胰岛素对严重烫伤后早期大鼠血乳酸浓度及乳酸脱氢酶活力的影响。方法40只SPF级SD大鼠,随机分为三组,分别是假烫伤组(8只)、烫伤组(16只)与胰岛素组(16只)。烫伤组与胰岛素组大鼠背部脱毛后置于(95±0.5)℃热水中15s,制作30%烫伤总面积(TBSA)Ⅲ度大鼠模型,伤后立即腹腔注射生理盐水(40mL/kg体质量)。假烫伤组37℃水中15s模拟烫伤。胰岛素组大鼠复苏补液后皮下注射胰岛素(1IU/kg体质量),烫伤组同法注射等量生理盐水。于伤后6、24h采集腹主动脉血。采用分光光度法测定血糖、血乳酸浓度及乳酸脱氢酶活力。结果①血糖:与假烫伤组比较,烫伤组伤后6、24h血糖显著升高(P〈0.05);与烫伤组比较,胰岛素组伤后6h血糖下降但差异无统计学意义(P〉0.05),伤后24h血糖显著降低(P〈0.05)。②血乳酸浓度与乳酸脱氢酶活力:与假烫伤组比较,烫伤组伤后6、24h血乳酸浓度、乳酸脱氢酶活力显著升高(P〈0.05);与烫伤组比较,胰岛素组伤后6、24h血乳酸浓度、乳酸脱氢酶活力显著降低(P〈0.05)。结论胰岛素干预能降低严重烫伤后早期大鼠的血乳酸浓度与乳酸脱氢酶活力。Objective To observation the effects of insulin on blood lactate (BL) and lactate dehydrogenase (LDH) activity in rats early after scald burn. Methods Forty SPF - grade Sprague - Dawley rats were randomly divided into three groups: sham scald group (8 rats), scald bum group (16 rats) and insulin group ( 16 rats). The rats in the latter two groups were subjected to 30% total body surface area (TBSA) full -thickness scald bum on the back. The rats in scald burn group were immediately resuscitated by intraperitoneal injection of saline (40 mL/kg body weight) ; the rats in insulin group were subcutaneously injected with insulin ( 1 IU/kg body weight) in an area of uninjured skin at the same time. The rats, abdominal aorta blood were collected at 6 or 24 hours after scald bum. The Blood Glucose (BG), BL and LDH activity were determined by spectrophotometry. Results ①BG: compared with sham scald group, BG in the scald burn group was significantly higher( P 〈 0.05 ) at 6 h or 24 h after scald bum; compared with scald burn group, BG in the insulin group was low but not significantly difference (P 〉 0. 05 ) at 6 h, and was significantly lower at 24 h ( P 〈 0.05 ). ②BL and LDH activity: compared with sham scald group, BL and LDH activity in the scald bum group was significantly higher(P 〈 0.05 ) at 6 h or 24 h after scald bum ; compared with scald burn group, BL and LDH activity in the insulin group was significantly lower(P 〈 0. 05) at 6 h or 24 h. Conclusion Insulin intervention may lower blood lactate and lactate dehydrogenase activity in rats early after scald bum.
