摘 要:目的:考察乌司他丁干预窒息大鼠心肺复苏后转录因子T-bet和GATA-3的表达。方法:随机将SD大鼠分为3组:假手术组(A组,不进行窒息及心肺复苏,仅进行麻醉和气管切开插管、血管穿刺);对照组(B组,静脉注射生理盐水4mL.kg-1);考察组(C组,乌斯他丁5万U.kg-1,给药容积为4mL.kg-1);采用实验设计模型考察大鼠复苏前后实验设定时间段血清T-bet和GATA-3mRNA表达及白介素-12(IL-12)、白介素-4(IL-4)、r-干扰素(INF-r)、肿瘤坏死因子-α(TNF-α)水平的变化。结果:B、C组复苏后6h分别与复苏后0h及A组比较,IL-12、IFN-γ、TNF-α水平均升高(P<0.01),B组复苏后6hT-bet表达上调,GATA-3表达下调,GATA-3/T-bet下降(P<0.05);与A组比较,C组复苏后6hT-bet表达上调(P>0.05),GATA-3表达上调(P<0.01),GATA-3/T-bet升高(P<0.05);复苏后6hC组与B组同时点比较,IL-12、IFN-γ、TNF-α水平低于B组,GATA-3表达上调、T-bet表达下调、GATA-3/T-bet升高(P<0.01)。结论:心肺复苏可对实验动物GATA-3和T-bet表达造成影响,并使其IL-12、IFN-γ和TNF-α水平升高,乌司他丁介入对此有一定的干预作用。OBJECTIVE To investigate the effect of ulinastatin injection on the disturbance of transcription factors T-bet and GATA-3 of the rats with anoxia-induced cardiac arrest-cardiopulmonary resuscitation(CA-CPR).METHODS Thirty SD rats were randomly divided into three groups:sham operation group(group A,n=6,only anesthesia,tracheotomy tube and vascular puncture);operation group(group B,n=6,anoxia-induced cardiac arrest-cardiopulmonary resuscitation and physiological saline 4 mL / kg was injected via vein);ulinastatin treatment group(group C,,n=6,anoxia-induced cardiac arrest-cardiopulmonary resuscitation and ulinastatin 50,000 U/kg was injected via vein);Factors including IL-12,IL-4,TNF-α and INF-γ and the mRNA expressions of T-bet and GATA-3 changes were observed at setting time before and after cardiopulmonary resuscitation in rats.RESULTS IL-12,TNF-α and INF-γ of group B,C were significantly increased as compared with those of group A(P〈0.01).In group B,the mRNA of GATA-3(P〈0.05) and GATA-3/T-bet(P〈0.05) were decreased while the mRNA of T-bet were increased(P〈0.01) in 6h after resuscitation.In group C the mRNA of GATA-3,T-bet and GATA-3/T-bet were increased in 6h after resuscitation.The concentrations of IL-12,IFN-γ and TNF-α in group C were lower than those of group B(P〈0.01).Compared with those of group B,the mRNA of GATA-3 and GATA-3/T-bet were significantly elevated while the mRNA of T-bet was decreased in group C.CONCLUSION Ulinastatin could upregulate GATA-3 expression of the rats after anoxia-induced cardiac arrest-cardiopulmonary resuscitation and down-regulate T-bet and decrease the concentrations of IL12,INF-γ,TNF-α.
