适形放疗加用CT导向下^(125)I粒子植入治疗盆壁复发肿瘤的近期疗效

摘　　要：目的评价适形放疗加用放射性125I粒子组织间永久性植入治疗盆壁复发肿瘤的疗效和安全性。方法对23例盆壁复发肿瘤患者(宫颈癌15例结直肠癌8例)共27个病灶采用计算机立体计划系统计算布源,在CT导向下行125I粒子立体周边插植植入术后一周内开始适形放疗治疗,结束后1-3个月复查CT。结果疼痛缓解率91.3%。下肢水肿缓解率90.9%。CR 56.5%,PR 34.8%,NC 4.3%,PD 4.3%。1例患者治疗区域肿瘤无变化对侧盆壁及腹膜后出现新生转移灶。结论放射性125I粒子植入术联合适形放疗治疗盆壁复发肿瘤安全、不良反应小,耐受性好。近期疗效满意远期临床疗效尚需进一步临床研究证实。【Objective】To evaluate the efficacy and safety of the conformal radiotherapy combined with permanent implantation of iodine 125 seeds for treatment of tumor recurrence in basin wall.【Methods】23 cases of patients with tumor recurrence basin wall（15 cases of cervical cancer and 8 cases of colorectal cancer of 27 lesions by using the permanent Implantation Therapy Treatment Planning System to calculate,undergoing CT-guided of 125I descending into three-dimensional particle around plant implantation,within one week after treatment began conformal radiotherapy,after 1～3 months to review CT.【Results】The pain relief rate is 91.3%.Leg edema ease rate 90.9%.CR56.5%,PR34.8%,NC4.3%,PD4.3%.1 case without change on the regional tumor basin wall and retroperitoneal appear newborn metastases.【Conclusion】The permanent implantation of iodine 125 seeds combined with conformal radiotherapy treatment basin wall cancer recurring safety,adverse reactions small,good tolerance.The long-term clinical curative effect with short-term curative effect needs further clinical studies confirmed.
