放射治疗联合替吉奥治疗局部晚期胃癌的临床观察

摘　　要：目的探讨放射治疗联合替吉奥(S-1)治疗局部晚期胃癌的临床疗效及不良反应。方法回顾性分析采用三维适形放射治疗联合替吉奥治疗局部晚期手术无法切除的胃癌患者33例,放射治疗:45Gy分25次,每天1次,星期一至星期五;同步口服替吉奥:80mg/m2,每天2次,星期一至星期五。结果患者治疗总有效率为63.6%,中位无进展生存时间为8个月,中位生存时间为16个月,1年生存率为72.7%,胃肠道反应和骨髓抑制是最常见的不良反应,未出现4级不良反应。结论三维适形放射治疗联合替吉奥治疗局部晚期胃癌是有效的治疗方法,且耐受性好。Objective To investigate the efficiency and toxicity of radiotherapy combined with chemotherapy with S-1 in-synchronism for advanced gastric cancer. Methods 33 patients with advanced gastric cancer inoperable but no metastasis,who were treated with 3D-CRT combined with chemotherapy with S-1 in-synchronism. Radiation was given to a total dose of 45 Gy,delivered in 25 fractions in 5 weeks by 3D-CRT,in-synchronism,oral chemotherapy with S-1 five days a week,daily 80mg/m 2 in twice,from monday to friday. Results The overall response rate was 63. 6% ,PFS 8 months,MST 16 months,The 1year survival rate was 72.7% ,Gastrointestinal reaction and hematological toxicity were the most commom toxicity,no reaction to 4 grade obviously. Conclusions 3D-CRT combined with chemotherapy with S-1 insynchronism shows good efficacy and tolerable toxicity in the treatment of patients with advanced gastric cancer.
