摘  要:目的:观察多系统萎缩患者的脑实质超声特征。方法:对多系统萎缩患者26例用经颅脑实质超声轴位检查中脑、第三脑室和侧脑室中央平面,对中脑黑质≥Ⅲ级的信号同时面积≥0.20cm2视为异常高回声,测量第三脑室宽度,检查基底节区有无异常高回声信号。同期检查30例健康人和非运动障碍疾病者作为对照组。结果:22例声窗正常的多系统萎缩患者进入分析,男12例、女10例;平均年龄62.4±11.7,病程4个月~7年。对照组30例,男16例、女14例;平均年龄61.8±8.2。多系统萎缩3例(13.6%)见到中脑黑质有异常高回声信号,与对照组比较无统计学差异。13例(59.1%)豆状核有异常高回声信号,与对照组有极显著差异。第三脑室宽度与对照组有显著差异。结论:经颅脑实质超声作为多系统萎缩型的辅助检查方法简便易行,结合临床应用有鉴别诊断意义。objective:To observe the characteristics of sonographic images of multiple system atrophy(MSA)by transcranial sonography(TCS).Methods:Using TCS to scan the midbrain,the third ventricle and lateral ventricle central plane by axial plane in 26 MSA patients.The cases whose substantia nigra echo intensity was≥Ⅲ grade and area was≥0.20cm2 were supposed to be hyperechogenicity.Additional,to measure the width of third ventricle and to find whether there is hyperechogenicity in basal ganglia.In the same time,we tested 30 healthy volunteers as the control group.Results:Twenty two MSA patients had normal temporal window.There were 12 males in this group,mean age 62.4±11.7 whose disease duration was from 4 months-7 years.Thirty volunteers had normal temporal window.There were 16 males in this group,mean age 61.8±8.2.In 3 MSA patients(13.6%),we could find hyperechogenicity in substantia nigra.There was no significant difference compared with the control group.In 13 cases(59.1%),we found hyperechogenicity in lenticular nucleus which had significant difference compared with the control group.Also,the width of third ventricle in MSA group had significant difference compared with the control group.Conclusion:TCS is a simple and easy method suitable for the application in the patients with multiple system atrophy.It can help clinical doctors to make differential diagnosis.
