摘 要:目的 研究老年性痴呆 (AD)患者和老年人松果体素昼夜和季节性节律变化。 方法 测定 85例确诊的AD患者和 82例无原发神经和精神疾病的老年人脑脊液中松果体素含量。 结果 AD组脑脊液松果体素水平为 (5 6± 7)pg/ml,明显低于老年对照组〔(2 73± 47)pg/ml〕。在老年对照组中 ,年龄≥ 80岁者松果体素水平为 (176± 5 8)pg/ml,比 <80岁者〔(330± 6 6 )pg/ml〕明显降低。对照组和AD组松果体素水平的昼夜节律均消失。短日照期内对照组老年人脑脊液松果体素水平明显高于长日照期 (P =0 0 4) ,分别为 (2 37± 5 8)pg/ml和 (138± 5 9)pg/ml。 <80岁对照组老年人脑脊液松果体素水平呈现季节性改变 ,冬季和秋季松果体素水平分别是春季的 3倍 (P <0 0 0 5 )至 7倍 (P <0 0 0 2 ) ,该季节性节律在≥ 80岁的老年对照组和AD组消失。 结论 松果体素水平及其昼夜和季节性节律紊乱可能是老年人和AD患者睡眠 觉醒周期等节律性行为紊乱的生物学基础。Objective To determine whether the changes of melatonin levels in postmortem cerebrospinal fluid (CSF) is dependent on the circadian and seasonal rhythms in Alzheimer's disease (AD) and the elders. Methods Melatonin levels were determined in CSF of 85 AD patients and 82 age matched controls. Results The CSF melatonin levels of AD patients were significantly decreased compared to those in control subjects 〔(56 ± 7) pg/ml vs (273 ± 47) pg/ml, respectively〕. In the elder control subjects (≥80 years of age), CSF melatonin levels were only the half of those control subjects that <80 years of age 〔(176 ± 58) pg/ml vs (330 ± 66) pg/ml, P=0.016〕. It was considered that there was no relationship between CSF melatonin levels and diurnal rhythm neither in control subjects nor in AD patients. In control subjects, the melatonin levels during short daylight period were significantly higher than that during long daylight period 〔(237 ± 58) pg/ml vs (138 ± 59) pg/ml, P=0.04〕. The melatonin levels in winter and autumn were 3~7 folds higher than that in spring (P<0.005 and P<0.002, respectively), suggesting a seasonal rhythm of CSF melatonin levels in <80 year old controls. Conclusions The present study showed a dissociation of melatonin levels with circadian and seasonal rhythms in the normal elderly people and AD patients over 80 years of age. This finding might contribute to the understanding not only for the sleep wake behavior, but also for other biological rhythmic disorders in elderly people and AD patients.
