奈达铂同步放化疗与顺铂+5-氟尿嘧啶同步放化疗治疗中晚期宫颈癌的疗效和安全性比较

摘　　要：目的:探讨中晚期宫颈癌患者在放疗同时进行奈达铂同步放化疗与顺铂+5-氟尿嘧啶同步放化疗两种治疗方案的疗效、不良反应。方法:选择中晚期宫颈癌患者86例,根据化疗方案的不同将86例患者分为NDP组(奈达铂同步放化疗法)和PF组(顺铂+5-氟尿嘧啶同步放化疗法)。观察与评价两种方案的疗效以及不良反应。结果:NDP组近期疗效明显优于PF组,NDP组远期的转移率和复发率均显著低于PF组,两组间的差异均有统计学意义(P<0.05)。术后3个月近期生存率和术后12个月远期生存率无统计学差异,两组间的差异无统计学意义(P>0.05)。NDP组Ⅲ～Ⅳ度的白细胞减少、Ⅲ～Ⅳ度的血小板减少、Ⅲ～Ⅳ度的恶心呕吐、Ⅲ～Ⅵ度的腹泻、Ⅲ～Ⅳ度的肾功能损害的发生率均显著低于PF组,两组间差异性均具有统计学意义(P<0.05)。结论:奈达铂同步放疗方案疗效优于PF方案,且不良反应特轻,病人耐受性较好,值得临床推广应用。Objective： To explore the curative effects and adverse reactions of nedaplatin（NDP） concurrent radiochemotherapy and cisplatin plus 5-fluorouracil（PF） concurrent radiochemotherapy for treatment of cervical cancer during the middle and late periods.Methods： Eighty-six patients with cervical cancer during the middle and late periods were selected and divided into NDP group and PF group;the curative effects and adverse reactions in the two groups were observed and evaluated.Results： The short-term curative effect in NDP group was statistically significantly better than that in PF group（P 0.05）,the long-term metastatic rate and recurrence rate in NDP were statistically significantly lower than those in PF group（P 0.05）.There was no statistically significant difference in postoperative three-month survival rate and twelve-month survival rate between the two groups（P 0.05）.In NDP group,the numbers of white blood cells and platelets in patients of degree Ⅲ-Ⅳ decreased,the incidence rates of nausea and vomiting,diarrhea,renal dysfunction in patients of degree Ⅲ-Ⅳ in NDP group were statistically significantly lower than those in PF group（P 0.05）.Conclusion： The curative effect of NDP regimen is superior to PF regimen,the adverse reactions are mild and the tolerance of patients is good,so NDP regimen is worthy to be popularized and applied in clinic.
