摘 要:目的:探讨微钙化灶在乳腺癌,特别是早期乳腺癌诊断中的价值。方法:收集58例的乳腺钼靶或超声图像显示微钙化灶且BI-RADS4级及以上的患者,对照其术后病理资料,评价乳腺癌术前钼靶和超声图像上微钙化灶的特征,包括微钙化灶的大小、形态、密集度、分布及与肿块的关系;分析超声及钼靶对乳腺癌的诊断率差异,对微钙化灶敏感性及与病理一致性的差异。结果:58例患者,病理证实乳腺癌54例,钼靶组诊断乳腺癌48例,超声组诊断乳腺癌40例。病理下可见微钙化57例,钼靶显示微钙化53例,超声显示微钙化47例。结论:超声对显示乳腺癌肿块及肿块内微钙化较钼靶有优势,钼靶对于肿块外钙化及以微钙化为唯一表现的乳腺癌优于超声,两者结合可提高乳腺癌,特别是早期乳腺癌的诊出率。Objective: To explore the significance of microcalcification in diagnosis of breast cancer,especially in early breast cancer.Methods: A total of 58 patients of BI-RADS4 grade or above with microcalcification showed by molybdenum-target photography and ultrasonography were collected,the postoperative pathological data were compared,the characteristics of microcalcification showed by molybdenum-target photography and ultrasonography before surgery were evaluated,including the size,morphology,density,distribution and the relationship with masses;the difference of diagnostic rates of molybdenum-target photography and ultrasonography for breast cancer and the difference of sensitivity of microcalcification and pathological coincidence were analyzed.Results: Among 58 patients,54 patients were diagnosed as breast cancer by pathological examination,48 patients were diagnosed as breast cancer in molybdenum-target photography group,and 40 patients were diagnosed as breast cancer in ultrasonography.Fifty-seven patients were diagnosed as microcalcification after pathological examination,53 and 47 patients were diagnosed as microcalcification by molybdenum-target photography and ultrasonography,respectively.Conclusion: Ultrasonography has advantages for displaying breast cancer masses and microcalcification in masses compared with molybdenum-target photography;for the patients with extra-tumor microcalcification and taking microcalcification as the only manifestation,molybdenum-target photography was superior to ultrasonography,joint application of molybdenum-target photography and ultrasonography can improve the diagnostic rate of breast cancer,especially early breast cancer.
