摘 要:目的研究个体化营养支持对急性重症脑卒中患者营养状况和临床结局的影响。方法采用随机数字法,将72例急性重症脑卒中患者按入院先后顺序随机分为个体化方案营养支持组(A组,n=37)和标准化方案营养支持组(B组,n=35),观察营养支持前后患者营养指标和空腹血糖的变化,住院期间感染并发症发生情况,住院时间和发生的费用。结果营养支持后,两组患者的体重、体重指数、三头肌皮褶厚度、臂肌围、GLU、CRP、和HB较支持前明显降低(P﹤0.01);两组患者的#LY较支持前明显升高(P﹤0.05);B组患者ALB和PALB较支持前有所降低(P均﹤0.05);B组患者肺部和肠道感染并发症明显高于A组(P=0.034,0.032),A组住院时间和住院费用明显低于B组(P均﹤0.01)。结论个体化营养支持可明显改善急性重症脑卒中患者的营养状况和血糖水平,其临床结局明显优于标准化方案营养支持组,适合在临床上推广应用。OBJECTIVE To study the effect of individualized nutritional support on nutritional status and clinical outcomes in patients with severe acute stroke. METHODS 72 cases of acute severe stroke were randomly divided into individualized nutritional support group (group A, n=37) and standardized nutritional support group (group B, n=35). Observed the changes of nutritional indices and fasting blood glucose in patients before and after nutritional support , the incidence of infectious complications during hospitalization and the hospitalization time and costs. RESULTS Two groups’ body weight, body mass index, triceps skin fold thickness, GLU, CRP and HB were markedly lower after nutritional support (P﹤0.01). The # LY was significantly higher after nutritional support, the difference was statistically significant (P﹤0.05). Group B patients’ ALB and PALB were lower after nutritional support (P﹤0.05). Group B patients’ lung and intestinal infection complications were significantly higher than group A (P=0.034, 0.032). Group A’s hospital stay and hospital costs were significantly lower than group B (P﹤0.01). CONCLUSION The individualized nutritional support can significantly improve nutritional status and blood glucose in patients with severe acute stroke, which clinical outcomes are significantly better than standard nutritional support group.
