摘 要:目的 了解经阴道超声照射对早期妊娠 (早孕 )妇女胚胎的安全性。方法 将 80例欲终止妊娠的早孕妇女分为 4组 (各 2 0例 ) :Ⅰ组为对照 ,不予超声照射 ;Ⅱ组经腹超声照射妊娠囊 10min ;Ⅲ组经阴道超声照射妊娠囊 3min ;Ⅳ组经阴道超声照射妊娠囊 10min。 1h后行人工流产术 ,取绒毛组织行电子显微镜超微结构观察 ,丙二醛 (MDA)、超氧化物歧化酶 (SOD)测定及姐妹染色单体交换率 (SCE)分析。结果 Ⅳ组绒毛断裂、丢失、排列不整 ,粗面内质网扩张 ,Ⅱ、Ⅲ组部分微绒毛排列紊乱、卷曲 ;SOD及MDA水平Ⅱ、Ⅲ组 [(2 1.0± 3.3)U/mg、(2 1.3± 3.6 )U/mg,(0 .39± 0 .13)nmol/mg、(0 .41± 0 .14)nmol/mg]与Ⅰ组 [(2 2 .9± 3.8)U/mg、(0 .36± 0 .0 9)nmol/mg]比较 ,差异无显著性 (P>0 .0 5 ) ;Ⅳ组SOD水平 [(13.1± 3.0 )U/mg]明显下降 ,MDA水平 [(0 .46± 0 .14)nmol/mg]明显升高 ,与Ⅰ组相比 ,差异均有极显著性 (P <0 .0 1)。Ⅱ、Ⅲ、Ⅳ组与Ⅰ组间SCE差异无显著性 (P >0 .0 5 )。结论 对早孕妇女应避免经阴道超声检查 ,宜采用经腹超声且不超过Objective To evaluate the effects of sonographic examination on embryo villi duing early pregnancy. Methods Eighty early pregnant women intended for artifical abortion were divided into 4 groups: group Ⅰ served as control ,the remaining 3 groups underwent pelvic sonographic examination transabdominally for 10 minutes (group Ⅱ),transvaginally for 3 minutes ( group Ⅲ ) or for 10 minutes (group Ⅳ) respectively. After 1 hour embryo villi were obtained through artifical abortion and examined by electronic microscopy,biochemical methods and sister chromatid exchanges (SCE). Results In group Ⅳ,but not group Ⅱ,Ⅲ, the embryo microvilli were found broken, lost and disarranged. The rough endoplasmic reticulum of trophoblasts expanded. Their malondial dehyde levels increased while the superoxide dismutase (SOD) decreased as compared with control ( P <0.01).There was no difference of SCE among these 4 groups. Conclusion It is recommended that sonographic examination should be done through abdominal approach for shorter than 10 minutes during early stage of pregnancy.
