胃癌胃镜活检标本增殖细胞核抗原的定量及其临床意义

摘　　要：目的　本文旨在探讨胃癌胃镜活检标本中增殖细胞核抗原的定量及其临床意义 ,尤其是对选定治疗方案的参考价值。方法　 4 1例源自甘肃省永登县 2 2 2 6例胃癌高发人群的胃癌入本组研究 ,对胃癌活检标本行增殖细胞核抗原 (PCNA)免疫组化染色 ,IBAS图像仪定量 ,并做术后生存率对比研究。结果　 4 0例手术治疗患者 1、2、3、4、5年的生存率分别为 2 2 5%、65%、52 5%、4 2 5%、35%。癌细胞中PCNA的含量与病变早晚呈正相关 ,与淋巴转移及远处转移正相关 (P <0 .0 5)。与术后生存期负相关 (P <0 .0 5)。结论　胃癌术前胃镜活检标本中PCNA的含量是胃癌恶性生物学行为的指标之一。术前行该检查可能成为围术前综合治疗 。Objective This paper is to investigate the expression of PCNA in gastric cancer from gastroscope biopsy and evaluate its clinical significance.Methods 41 cases with gastric cancer,from 2226 cases highrisk population in Yunden County of Gansu Province,were studied.Gastroscope samples were assessed by immunohistochemical study using a monoclonal antibody to PCNA(PC-10)and quantitated by IBAS image analysis technolog,All of cases were followed up and correlated with PCNA expression.Results The 1-,2-,3-,4-,5-,year survival rate were 72.5,65.0,52.5,42.5,35.0 Percent respectively.The value of PCNA was positively correlated with TNM stages system and metastasis( P <0.05),negatively with long time survival.Conclusion The PCNA quanty is one of the malignant biologic behavior indexes.Perioperation examination of PCNA could provide reference for choosing comprehensive regimen especially for induced chemotherapy.
