区域性胰腺切除治疗胰腺良性肿瘤  

Clinical application of regional pancreatectomy for benignlesions

摘　　要：区域性胰腺切除术因切除的胰腺组织少，保留了胆总管、十二指肠、脾脏和有功能的胰腺，患者的创伤较小，术后并发症发生率较低，对胰腺内、外分泌功能影响较小，近年来逐渐被应用于胰腺头、颈、体部良性肿瘤的治疗。本研究回顾性分析2010年1月至2011年12月厦门大学附属第一医院19例施行区域性胰腺切除术患者的临床资料，其中12例行保留十二指肠和胆总管的区域性胰头切除术，7例行胰腺中段切除术。术后随访3-48个月，无胰瘘、胆瘘和胃肠道并发症发生。因此，区域性胰腺切除术可作为胰腺头、颈、体部良性肿瘤的首选术式。Regional pancreatectomy has been gradually used in the treatment of benign lesions in the head, neck and body of pancreas because of less removal of the pancreas and preservation of the common bile duct, duodenum, spleen and more functional pancreas. Regional pancreatectomy is less inva- sive and reduces the incidence of postoperative complications. The clinical data of 19 patients who received regional pancreate- ctomy at the First Affiliated Hospital of Xiamen University from January 2010 to December 2011 were retrospectively analyzed. Of the 19 patients, 12 received regional pancreatectomy of the bead of pancreas with preservation of duodenum and common bile duct, and 7 received medial pancreatectomy. All the patients were followed up for 3-48 months, and no complications including pancreatic fistula, biliary fistula or gastrointestinal complications was found. Regional panereatectomy could be used as the optimal approach for the treatment of benign lesions of the pancreas.
