摘  要:目的探讨MSCT多期动态增强扫描检测肾细胞癌(RCC)假包膜的价值。方法回顾性分析80例经手术病理证实为RCC的患者的影像学及病理学资料。对所有患者术前行中腹部16层CT或320层CT平扫和多期动态增强扫描,并行MPR;以病理学结果为金标准,评价MSCT显示RCC假包膜的能力。结果 29例RCC经病理确诊存在假包膜。应用MSCT多期动态增强扫描,26例可清晰显示假包膜,其诊断敏感度、特异度、阳性预测值和阴性预测值分别为86.21%(25/29),98.04%(50/51),96.15%(25/26)和92.59%(50/54);排泄期为显示RCC假包膜的最佳时相,其中冠状位和矢状位MRP显示RCC假包膜与病理结果一致性较高(Kappa值≥0.7)。结论 MSCT多期动态增强扫描可较准确地显示RCC的假包膜。Objective To explore the value of multi-phase dynamic contrast enhanced MSCT on detection of pseudocapsule of renal cell carcinoma (RCC). Methods Imaging and pathology data of 80 patients with RCC confirmed pathologically were retrospectively analyzed. The patients underwent 16-slice or 320-slice unenhanced and three-phase contrast-enhanced CT scanning, and all the raw data of CT were transmitted to workstation for MPR. The capability of MSCT for detecting pseudocapsules of RCC was evaluated taking pathological results as golden standards. Results There were 29 cases of RCC with pseudocapsule identified by pathology. In MSCT, 26 cases of RCC with pseudocapsule were observed, the sensitivity, specificity, positive predictive value and negative predictive value of MSCT for detection of pseudocapsule of RCC was 86.21% (25/29), 98.04% (50/51), 96.15% (25/26) and 92.59% (50/54), respectively. Imaging in the nephrographic phase with coronal and sagittal MPR was proved more accurate in detection of pseudocapsule, with high consistency to pathologic results (Kappa≥0.7). Conclusion Multi-phase dynamic contrast enhanced MSCT can display pseudocapsule of RCC accurately.
