摘　　要：目的探讨CT检查对胃平滑肌瘤的诊断价值,为胃平滑肌瘤的诊断提供参考。方法以56例经CT检查术前初诊为胃平滑肌瘤患者作为研究对象,总结其CT表现,分析CT诊断准确率、误诊率,并与胃镜检查进行对比。结果胃平滑肌瘤CT表现为大小(12.6±9.8)mm的软组织密度肿块影,边界清楚,形态规则。CT诊断准确率为78.6%(44/56),误诊率为21.4%(12/56);胃镜诊断准确率为78.9%,误诊率为21.1%,两者相比无显著差异(χ2=0.002,P=0.965),但胃镜漏诊10例。CT对胃内型平滑肌瘤误诊率为22.2%,高于胃镜检查(12.5%),但两者无显著差异(χ2=1.102,P=0.294)。CT对胃壁型平滑肌瘤的误诊率为16.7%,显著低于胃镜检查(66.7%)(χ2=4.500,P=0.034)。CT对胃外型平滑肌瘤的诊断优于胃镜检查。结论 CT可显示胃平滑肌瘤的大小、形态、位置、密度,并且不受病变位置的限制,对胃平滑肌瘤的诊断和鉴别诊断意义重大。Objective To explore the value of CT examination in the diagnosis of leiomyoma of stomach. Methods Fifty six patients diagnosed as leiomyoma of stomach by CT examination were selected as subjects. Summary was made in their CT manifestation. The diagnostic accuracy rate and misdiagnosis rate were analyzed and compared with those of gastroscope. Results The stomach leiomyoma showed a lump shadow of soft tissue density with the size of （ 12.6 -＋ 9.8 ） ram, clear boundaries, and regulate shape. The diagnostic accuracy rate of CT was 78.6% （44/56） ,and the misdiagnosis rate was 21.4% （12/56）. The diagnostic accuracy rate of gastroscope was 78.9%, and the misdiagnosis rate was 21. 1%. There were no significant difference between the two examination methods （X2 = 0.002,P = 0. 965 ）, but 10 patients were missed diagnosed by gastroscope. The misdiagnosis rate of CT for inner gastric leiomyoma was 22.2% ,which was higher than that of gastroscope （ 12.5% ） ,but there was no significant difference （X2 = 1. 102 ,P = 0. 294）. The misdiagnosis rate of CT for leiomyoma of gastric wall was 16.7% , which was significantly lower than that of gastroseope （ 66.7% ） （X2 = 4. 500, P = 0. 034 ）. The diagnosis by CT was better than that by gastroscope. Conclusion CT can show the size, shape,location, and density of the stomach leiomyoma. It is not restricted by the tumor location. So it plays a great role in the diagnosis and differential diagnosis of stomach leiomyoma.
