摘 要:目的对肝脏和肝血管进行三维重建可视化并进行肝分段,探讨其意义及临床应用。方法利用Mimics软件对24例晚期弥漫性肝癌患者CT扫描数据进行图像分割并进行三维重建,根据每个患者肝血管的分支和Couinaud分段法对肝脏进行个体化分段,计算肝和各肝段的平均体积。结果重建后获得形态逼真的肝脏和肝血管的三维模型,测得患者全肝平均体积达(2683.26±671.53)cm3。对肝脏进行了分段,通过三维模型能准确计算出各肝段的平均体积。结论三维重建、肝脏分段和体积测量为患者的诊断、后续治疗和手术风险评估提供了参考,来源于活人体的三维图形也可以用于科研和教学。Objective To perform three-dimensional reconstruction visualization and segmentation on liver and liver blood vessels and to explore their significance and clinical application.Methods Mimics software was used to perform image segmentation and three-dimensional reconstruction on the CT scan data of 24 patients who were diagnosed with advanced diffuse hepatocellular carcinoma.Individualized segmentation was performed according to each patient ’ s hepatic artery branches and the Couinaud segmentation method,and the average volume of the liver and each segment were calculated.Results Vivid three-dimensional models of liver and liver blood vessels were constructed through three-dimensional reconstruction.The average volume of the whole liver was(2683.26±671.53) cm3.After segmentation,the exact average volume of each liver segment could be calculated from the three-dimensional model.Conclusion Three-dimensional reconstruction,liver segmentation and volume measurement provide references for the diagnosis,follow-up care and surgical risk assessment of patients.Three-dimensional graphics derived from the living body can also be used for research,teaching and learning.
